I, JELLEN L. LORETO, of legal age, Filipino, married and a resident of

Sitio Sta. Lourdes, Brgy. Behia, Magallanes, Sorsogon after having been duly
sworn in accordance with law, hereby depose and say;

1. That I am a registered member of the Foundation for

Advancement Thru Integrated Techniques for Humanity (FAITH), Inc.,
Sorsogon Branch with business address at Brgy. 13, 7th Street Patag District,
Catbalogan City;
2. That I have paid the amount of Php 550.00 per week for four
weeks already thus I have already paid the total sum of PhP 2,200.00 and it
was received by the representative/collector of FAITH, Inc. known to us by
the name of “ROSALE”
3. That I was not issued receipt relative to it;
4. That I hereby attest that I indeed paid FAITH, Inc. for the
obligation above indicated;
5. That I am executing this affidavit to attest to the foregoing facts.
