COMPLAINT AFFIDAVIT

I, Albert Hipolito, 56 years old married a businessman and a resident of 333 San Juan,
Banio, Arayat, Pampanga, after being sworn in accordance with law, depose and state
that:

1. That in the morning of January 8, 2017, I received text messages for a group
claiming to be members of the of New People Army of Arayat. Pampanga asking
revolutionary tax of P 100,000.00 cash and they said that they will burn my gasoline
station if I will not abide with their demand.

2. That the suspects give me specific instruction to put the cash in a brown envelope
which they will pick up on or about 12:00 pm of the same date at my gasoline
station (Jetti Gasoline Station, Mc Arthur Highway, Brgy. Tagulud, City of San
Fernando Pampanga);

3. That on or about 10:00 am in the morning of January 8, 2018, I voluntarily went to

the Police Station 2 of City of San Fernando to give the necessary information and
oath regarding the “Robbery Extortion” I experienced against suspects Alfredo
Dalisay, y Abot 46 years old, single, unemployed and a resident of 047 Culubong
San Matias Sto, Tomas Pampanga, and James Reyes, y Raot, 28 years old
unemployed and a resident of St. Francis Village, Dolores, City of San Fernando
Pampanga.

That I have executed this affidavit to attest the truthfulness of the foregoing facts
