AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This agreement made and entered into by and between the following parties, to
wit:

For a Unique Style of Entertainment, represented by RONAN R. CAPALA, of

legal age, Filipino and with address at 1990 Almeda St., Tondo, Manila, hereinafter
referred to as the FIRST PARTY:

Nenette Banaag, of legal age, Filipino and with address at 13 Garnet Rd. Pilar
Village, Las Pinas City, hereinafter referred to as the SECOND PARTY.

WITNESSETH : That –

WHEREAS, the SECOND PARTY intends to hire the services of the FIRST
PARTY for Mommy Glo’s 75th Birthday Celebration to be held at BF Pilar Southville
Clubhouse, Pilar Village, Las Pinas City on July 12, 2014, 6PM.

WHEREAS, the FIRST PARTY agrees to perform at the said occasion to be held
by the SECOND PARTY;

NOW THEREFORE, for and in consideration of the foregoing premises, the

PARTIES hereby agree as follows:

1. The FIRST PARTY shall perform on JULY 12, 2014, 6 PM for TWO (2)
set(s) (divided into ONE HOUR every set).

2. For the said performance, the SECOND PARTY shall pay the FIRST
PARTY the amount of FOURTEEN THOUSAND PESOS
(P 14,000.00) in cash for the talent fee payable as follows:

P 7,000.00 as DOWN PAYMENT and P 7,000.00 at the end of the show

performed by the FIRST PARTY for the COMPLETION of the contract.

P 3,500.00 as ADDITIONAL FEE for every 30 MINUTE EXTENSION,

3. It is agreed by the parties herein that the SECOND PARTY shall provide
the food and venue and the FIRST PARTY shall provide the sound system
[SPECIFICATIONS: ACOUSTIC BAND SETUP] for the show and
shall also be responsible for their musical instruments, costumes and
make-up or any other props that they might need in their performance.

4. In case the show is cancelled before the agreed date of show for whatever
reason not attributable to the fault or negligence of the FIRST PARTY, the
SECOND PARTY shall pay the FIRST PARTY half the amount of the
talent fee (P 7,000.00) agreed upon.

5. The parties have read this contract and understood fully all the provisions
contained herein. Any amendments/revisions/additions shall be reduced in
writing and signed by the parties.
