Introduction

Objective Making a contribution to safe working, by:


1. Requiring the express permission of the Functional Owner (or his deputy),
before the works can be started up;
2. Making sure that the risks of the works have been explored in advance, and
that the measures that are a consequence of this examination have been
clearly communicated to the workers.

Risk identification and The basis for the measures that are applicable to the work permit procedure:

 Operational procedures (general and/or specific);
 Hazard cards;
 Risk analyses;
 Indication of hazards in the plant.

Application area All works that are carried out on the terrains of:
 Antwerp Refinery;
 Antwerp Depot.
IN CASE OF EMERGENCIES (namely in case of a gas or a fire alarm):
 Expiry of all authorizations and permits
 Works may be carried out without a work permit, at the orders of the
Incident Controller or the Fire Marshal.

 A work permit may be issued by a senior foreman, at the request of


SSI/DSS (see Chapter 3).

Application area 1. Report: (See Chapter 1)


 Activities (controls/inspections) during which the risks remain limited until
entering the the process unit;
 Verbal agreement is required.

2. Notification work permit: (See Chapter 2)


 Small routine works with a limited risk;
 Written permission on the printout of the notification is required.
3. Work permit: (See Chapter 3)
 All works (incl. transport) where the risk is that high so that notification
work permit is not sufficient;
 A written permit is required, for which an application must be made via the
standard work permit form.
4. Special procedures: (See Chapter 4)

1. Reporting obligation

Definition Reporting at a processing unit in order to obtain verbal permission for entering the
unit, for works with a very limited risk (see section on "Type of works").

Type of works All works that do not fall under a notification work permit and for which a work
permit is not required.
In other words, works that can be carried out without additional safety measures,
as described in the left-hand panel (Mechanical) and/or in the right-hand panel
(Process) of the work permit form.
Examples:
 Routine checks of equipment by permanently employed personnel, such as
machinists, electricians, etc.;
 Topographical works at ground level;
 Exterior visual inspections;
 Emergency tests (continuous escort by process unit personnel);
 (Re)certification of scaffolds.

Additional Additional rules for entry of H2S hazard zones that have been cordoned off (see
requirements SP-B1).

Applicant Everyone that wants to enter a section of the factory without a permit.
Exception: Console-linked personnel.

Person granting Operator / foreman of the corresponding unit.

Location Inplant house of the corresponding processing unit or WPC.

Procedural steps  The applicant reports to the operator or foreman of the corresponding unit and
provides information on:
 The reason for the visit;
 The probable duration of the visit;
 The number of visitors.
 Onsite: If the operator or foreman is not present, the person can report to
IPH via an installed radio communications device.
 The person issuing permission can subsequently give verbal approval to
access that section of the factory.
 The applicant must report out at the conclusion of the visit, at the same place
where he had reported in.

Administration If a visit takes place during a change of shift, then the person issuing permission
has to record this in his shift transfer logbook (SO-ONS-1-2B).

2. Notification work permit

Definition Permission in writing (= permit) to carry out small and routine works with a limited
risk (see the section "Type of works"). In case of doubt, see Functional Owner.

Type of works Works:


 Printout of notification work permit, which is only valid for a single working
day; If the works remain "in progress" then the notification work permit
must be renewed each day;
 During which only simple manual tools are used;
 For which no special security measures are required;
 Which have a limited complexity;
 Which are carried out by workers acquainted with the installations.
 Examples (non-limitative):
 Painting (pipelines, structures, buildings, etc.);
 General transport (not on the roads of the processing unit or in high
risk areas);
 General housekeeping (cleanup, tidying up of materials);
 Visual inspection (including dye checks, thickness measurements,
etc.);
 Surveys;
 Small civil works (for instance the positioning of sleepers, repair of
local roads);
 Maintenance of the lighting facilities (e.g. replacement of lamps);
 Disconnection and connection of instrumentation / analyzers that are
not safety-critical (see exceptions);
 Use of a battery-operated screwdrivers (see exceptions);
 ……

Exceptions Also permitted with a notification work permit in non-high risk areas, on the
condition that continuous LEL detection is used:
 IMEA maintenance works:
 Use of electronic measuring or calibration equipment (battery
operated), which are not explosion-proof;
 Opening of an explosion-proof (EExd, EExp, EExe) box;
 Screwing in and screwing out screws with a battery-operated
drilling machine that is not explosion-proof.

In the event that the LEL detector gives an alarm:


 All works must be immediately stopped (switch off all
equipment) and/or close the explosion-proof box;
 Immediately inform the process unit foreman.

Works that are never Works:

 Based on the type of works to be carried out, the applicant


Procedural steps Application checks whether these can be carried out with a notification work
permit.
 The applicant prints out 2 copies of the notification work permit
or fills in a pre-printed form, which is handed over to the worker.

Control  On the basis of the notification work permit, the issuer of the
permit checks whether the conditions for carrying out the works
are fulfilled.
For I&A notification work permits, this control will be carried out
by the Console Foreman.
The Functional Owner makes spot checks, to see whether the
notification work permit is being correctly applied.

Validation of the  The issuer of the permit makes sure that the recipient
notification work understands the works, so that these can be carried out safely.
permit  The name and signature of two persons is required for validating
the permit.
 Issuer: Gives permission to start the works, after he has
assured himself that the works can be carried out safely.
 Recipient: Has taken notice of / confirms the prescribed
measures with his signature.
 1 copy of the permit will contain the name of the issuer and the
recipient, who both sign the permit; this will be stored at the
work location. This copy must always be provided with a SWM
(StartWork Meeting) section.
StartWork  The recipient of the permit transmits the notification work permit
to the worker.
 Before an implementation of the works is started, the
worker organizes a short SWM and fills in the StartWork
form on the notification work permit.
Follow-up  1 copy the notification work permit will be kept by Process as
information (notification work permit file); this copy does not
have to be signed by both parties. This copy remains on file in
the Inplant house, as long as the works are in progress.
Signing out  If relevant for Process: Description on the notification work
permit of the repair that has been carried out or the state of the
works.
 Signing out can occur at the Inplant house, or per radio or
telephone.
 If the work has not been finished, and if the process unit
personnel (issuer) it is not present, then the worker will take the
copy of the permit out of the file and record on it the state of the
works (signing out), and then put the signed out copy of the
permit back into the file.
 After signing out, all copies are reviewed by the process unit
personnel (issuer) and any operational steps that are still
required will be taken.
 After signing out, and if no additional steps are required, then
the copies of the permit are removed from the file by the issuer.
 Notification work permits that have not been completed remain
"in progress". If all works have been completed, then the
recipient of the permit, or the Esso contact person, will mark the
notification work permit as "completed". In both cases, the
status/cause/feedback will be noted down in SAP.

3. Work permits

Types  Cold work permit (green);


 Hot work permit (red);
 Transport permit (ocher-yellow copy of the cold or hot work permit);
 Tank permit (= combination of entry permit (incl. area release) + work release).

Term of validity  Day permit;


 Multiple-day permit - the use of such a permit is only permitted under specific
conditions (see below).

Specific conditions  The works and the location must be precisely described (possibly on the basis
for multiple-day work of a supplementary list of details or notification work permits, such as
permits insulation works / scaffold construction);
a copy of his detailed list or notification work permit must always be present at
the workplace and in IPH. The process unit makes an assessment whether an
advance field verification is required, for guaranteeing that:
- Access to an operation of the process unit remains assured;
- That the surroundings do not pose any risks for carrying out the works.
 The condition and the surroundings of the equipment are not subject to any
changes, which have an impact on a safe implementation of the works;
 Type of works:
 Cold works: (no additional limitations);
 Cold works with LEL detection; with the exception of High Risk Areas;
 Hot works: Only in process units that are not operational and gas-free;
 A multiple-day work permit may be used for carrying out cleaning
works in a tank;
 Hot works for vacuum vehicles on Offsite, on the condition that:
- The permit is accompanied by a drain list, on which the Foreman
has indicated the tanks that may be drained each day;
- An LEL detector must be continuously present;
- Vacuum vehicle personnel must have a radio at their disposal,
and at least 1 or both operators of the vacuum vehicle must have
followed supervisor training at OM&S (training data, incl. names
of the persons, must be included in SOP);
- The vacuum vehicle always remains on major roads;
- Additional works (emptying of barrels, vacuum-cleaning of spills)
may only be carried out at the request of the process unit.
Examples: Paintworks, non-destructive measurements, maintenance of flares,
working on a tank that is not operational, repair of fencing, measuring of tanks,
greenery maintenance, etc.

3.1 Procedural steps

Application Filling in the Mechanical section (= left-hand panel) and attaching possible
additional permits, RA, checklists and attachments for the tank permit.

Meeting  Onsite: Process Services (Senior Foreman for the day);


(before 2 p.m.)  Offsite + Depot: WPC;
 Other: Functional Owner (or deputy).

Handling Process Services / WPC will obtain work permits including


eventually required explanations  to the Functional Owner.

Approval for further The Functional Owner (or his deputy) checks the work permit application and
handling signs it for further handling.

Process unit Objective Operationally securing the process equipment, so that the
preparations planned mechanical works can be carried out safely.

Implementation  Process unit nightshift (possibly late shift): Indication of the


'necessary' process unit preparations for the work permit
and starting with the necessary preparations;
 Morning shift (before 7:45 a.m.): Implementation /
controlling the 'necessary' process unit preparations and
completing the Process Unit section of the permit.
 Antwerp Depot: Necessary preparations by TT, before the
start of the early shift.
Deviation In urgent cases, the determination, preparation and control of
the 'necessary' process unit preparations can be carried out by
the same duty.

Multiple-day The process unit section of the permit must be supplemented


permits each day (gas tests / condition of the surroundings / etc.).

Validating a work The name of the issuer of the work permit must be filled in (and possibly the
permit Senior Foreman / Functional Owner) and of the recipient of the work permit
(responsible worker).

Multiple-day Such a permit must be revalidated each day.

Issue of work permit  The recipient can pick up his work permit in the IPHs (onsite and offsite):
 In principle between 7:45 a.m. and 8:15 a.m.;
 In exceptional cases after 8:15 a.m.
 The work permit must always be stored at the location of the work.
For works in enclosed spaces: The work permit is hung up at the manhole,
together with the 'entry permit'.
 Tank permit (entry permit (incl. area release)) will be left behind in a provided
permit box (before 7:45 a.m.) by WPC, after a gas measurement has been
carried out. A work release, for each contractor and each tank, will be
confirmed by Offsite Mechanical on the preceding day. The tank permit
consists of a combination of both documents (entry permit incl. area release
AND work release).

StartWork  The recipient of the permit transmits the work permit to the worker.
 Before execution of the works is started, the worker organizes a short SWM
and fills in the StartWork form on the notification work permit.

Supplementing a If unforeseen help of another trade is necessary for execution of the works, then
work permit the work permit can be supplemented.
The unit foreman supplements the works/measures on the worker's copy of the
permit and signs it.

Extension of work  Requests must be made before 15:30 p.m. to the (day) Senior Foreman of
permit the process unit, WPC.
 The Functional Owner must be informed about the duration of the extension.

Interruption of work The works covered by the permit must be interrupted, whenever the process
permit foreman / worker considers the working situation unsafe.
In this case, the issuer of the permit takes back the work permit from the worker.
 Work may only be resumed after permission has received from the issuer of
the permit.

Termination of work The work permit is terminated by returning it to IPH or WPC.

Follow-up on work 1. WPC: Daily overview list per console of the requested work permits (only
permits Offsite).
Overview list of locations: According to the agreement in PMT.

2. Process Foreman: Daily filing in every IPH of all active (i.e. issued) and non-
active (i.e. not issued or returned) work permits.
Objective: A quick and efficient monitoring of the status of the works.

3. Information in the event of a shift transfer (see SO-ONS-1-2B); Double


responsibility:
 Foreman of the departing shift: Must inform his successor at least of all
critical work permits (hot work permits and important cold work
permits);
 Foreman of the upcoming shift: Checks the status of the work permits
in the general file, with special attention for specific guidelines on the
permits.
Critical work permits must be checked onsite.

4. Process Services / WPC: Weekly collection of the completed work permits


(original copy of the terminated work permit, white copy IPH, SWM) for filing
(1 month).

3.1.1 Routing specific for tank permits

BASIC PRECONDITION:
After a tank has been taken out of service and cleaned, the SECTION HEAD must sign off on the following checklists
(see SP-F35):
- Tank cleanliness list, before the start of mechanical works;
- Release of the tank and the tank area (plotter plan of the involved area, including operative equipment (piping,
sewers, etc.) indicated on the map)).

APPLICANT: Requests an entry permit (incl. area, which CONTRACTOR: Requests work release (see tank permit

FO/WPC: Carry out gas tests and evaluate the


surroundings.
If gas tests and surroundings are OK  validate the entry
permit and hang it up in the permit collection box.

CONTRACTOR: Checks the available entry permit for the approval date and signs the entry permit for confirmation. As
an additional confirmation that a check of the entry permit has been made, and that the work release document has
been reviewed, the contractor also signs as the recipient on the work release. This means that if an entry permit is
available locally AND the contractor disposes over a validated work release permit, then the works can be started. If 1
of either of these documents is not in order or available locally, then the works cannot proceed. A StartWork
meeting has to be held before starting with the works.

ON COMPLETION OF THE DAY TASK: Main contractor brings in the permit to Operations. Work release: If validated
for the following day  then the work release should be used the following day; if no validation for the following day 
then the work release should be returned to Mechanical.

2 Types of permits

3.2.1 Cold work permit

Type of works  All cold work that does not require a notification work permit or a verbal
agreement may be carried out.
 Also permitted with a cold work permit are works in non-high risk areas, on
the condition that continuous LEL detection is used:
 Use of electronic measuring or calibration equipment (battery
operated), which is not explosion-proof;
 Opening of an explosion-proof (EExd, EExp, EExe) box;
 Disconnecting or connecting live cables of transmitters, solenoids,
contact switches, etc. (non-EExi version);
 Screwing in and screwing out screws with a battery-operated
drilling machine that is not explosion-proof;
 Taking photos with a camera.

In the event that the LEL detector gives an alarm:


 All works must be immediately stopped (switch off all
equipment) and/or close the explosion-proof box;
 Immediately inform the processing unit foreman.
 Exception: taking pictures with a camera requires a special photo permit and
written permission by Security (see 4.8).

Special, supplementary permits and control sheets: (See Chapter 5).

3.2.1.1 Responsibilities and tasks linked to functions

Work permit applicant Authorized functions  Operational Esso departments: Mechanical,


Construction and Inspection.
 For small works: In specific cases, the Esso
functional owner.
 In exceptional cases, and after training and approval
through the functional line management and the
Mechanical support manager:
- Esso equivalent function (hired or borrowed
supervisors);
- Supervisors of the external project
organization;
- Engineering offices;
- Third-party working on its own installation.
 After approval by the functional line manager and the
Mechanical support manager, the permit is written by
the contractor under consultation with the job leader.
Job leader (Esso) examines and signs the permit,
and continues to remain responsible.

NB: The Safety Department assures training and follow-


up.

Responsibility  Correct and complete specification of:


 The works;
 The hazards / risks on the basis of the
planned works;
 The measures required for managing the
hazards / risks of carrying out the works.
 A timely discussion of the application with the
Senior Foreman of the day of the process unit, ATM
or WPC.
 Works must be discussed in advance with the
person responsible for carrying out the works.
The applicant then determines whether this can be
carried out in the field or not.
 Guarantees for quality and safety of the works.

Work permit applicant Tasks  Complete specification of the works in the header.
If necessary, a sketch or a detailed list should be
added (for instance for insulation works, scaffold
construction, etc.).
 In the event of complex works, it is recommended
that a digital photo or printout of the 3-D model is
attached to the permit application.
 Planned works should be checkmarked in the
checkbox 'cold works'.
 The field 'Transport of machines' must be filled in, if
applicable.
 Checkmark hazards / risks that are typical for the
works being carried out (possibly on the basis of an
RA).
 Checkmark whether an additional permit or control
sheet is required and add these to the work permit
application.
 Opening/cutting of operational equipment must be
accompanied by a VOS document (verification
sheet for opening and cutting of operational
equipment in SP-C4).
 Tank permit:
 Completion of the entry permit (incl. area
release);
 Preparation of a work release for each
contractor (do not send to the process unit but
directly to the contractor).
 Check that all works, for which an application is
made, can be carried out together.
 If applicable, checkmark the checkbox 'Risk
Analysis' (+ report or ref. no.) and attach the RA to

The most important points of attention and


measures for the workers must be taken over from
the RA into the Mechanical section of the work
permit.
 The RA must be available both at the workplace
(together with the work permit) and in the IPH (filing
together with work permit)!
Comment: In the event that a very detailed RA
is available, then it is recommended that
only that part is kept at the workplace, which
is applicable at that time.
 Refer to specifically relevant SPs.
 Report the numbers of other work permits that may

 The 'issuer' of the permit must sign off when a worker


reports a job as completed, either as an
acknowledgement of the completion or for a field check.
With this the unit foreman confirms that the work permit
has been returned and that he has made an independent
decision, as to whether a field check was necessary or
not.
Examples of where a field check is necessary: -
Dryer, after salt was replaced;
- Pump, after reinstallation;
- Valve, after replacement, etc.
In some cases it is recommended that the field check is
carried out together with the worker.
A field check is not required when the Mechanical works
have not yet been terminated, or after most of the IEA
works (for instance checkup of analyzer, replacement of
a transmitter, and works in a substation).

3.2.2 Hot work permit

Basic requirements Cold work permit (see 3.2.1);

Type of works All works that can generate sufficient energy (sparks) or sufficient heat for an
ignition, and which do not fall under a cold work permit (see the section 'Hot
works' on the hot work permit).

Risk analysis In addition, a risk analysis is required for the following cases:
 Activities in zones with a higher probability for the release of flammable
products (high risk areas => see SP-A5).
 Unsecured installation of equipment that is not explosion-proof, or which does
not fulfill the zoning requirements.

Possible hazards The energy required for igniting flammable hydrogen or hydrocarbon mixtures is
so small that every spark can cause ignition.

Fire watch See the task description in SP-A2, if a fire watch is required.

Shielding of hot work Whenever sparks or hot metal particles can freely penetrate into the larger
zone surroundings, then the hot work zone must be screened off (SP-B21).

B913, Industriedok, waiting dock, etc.

Basic requirements Hot work permit (see 3.2.2).

Additional Waiting dock: Separate permit to be requested from the harbor captain's service
requirements (fax 03 205 20 25). In urgent cases, permits for docking locations can be
requested by telephone from the harbor captain (tel. 03 205 21 83), and outside of
regular business hours from the standby harbor captain, via the works supervisor

be sent immediately afterwards to the shipping section of the harbor captain's
department.
All docks, including the waiting dock: Moored ships must be informed of hot
works that must be carried out at less than 20 m from their location.
A periodical permit for hot work is applicable to all docks, with the exception of the
waiting dock; this permit must be renewed biannually.

Extra tasks

Work permit applicant Task For works at the waiting dock: A permit must be applied
for at least 2 days in advance from the Functional
Owner.

SOI Task For the waiting dock: Application for a permit must be
made with the harbor captain's department at least 1
day in advance of the planned works.

4.7 Work permit procedure for unloading catalyst/chemicals

For unloading chemicals/additives, and for unloading (and loading) catalysts, the permit consists of the forms
that are attached to SERV-115 (page 6).
1. Permit for unloading chemicals - bulk deliveries
2. Permit for delivery of chemicals - drums
3. Permit for unloading and loading catalyst and chemical packages
4. Unloading of chemicals after office hours

These are white forms that, besides an explicit unloading permit, also contain a checklist about documents,
free space required for unloading, the state of the equipment and personal protective equipment. Details are
described in SERV-155. The forms are signed by the Chemicals Officer and the truck driver.

In the event that unloading proceeds within a processing unit (inside the yellow lines), then Process
must be informed and must countersign the permit ("unit foreman" field) before unloading can
proceed.

If the help of riggers is required for unloading catalyst from a truck, such help will be requested by way of a
notification. The "permit" field in the notification will make reference to the form SERV-155.

4.8 Safety photo permit

Type of work What: because of safety concerns, taking photographs of Esso equipment, buildings and
operational installations is subject to strict regulation.
Where: these rules apply to all photography within Esso grounds.

Basic requirements  A valid security photo/filming permit.


 A signed safety photo permit.

Additional requirements  LEL detector.


 No battery changes in the plant.
 Hot work permit required for photography in high risk areas.
