AFFIDAVIT OF WARRANTY/UNDERTAKING

I, JOEL B. FERRER,of legal age, and residing at 23 Jones cor. Kent Streets, Filinvest East,
Cainta, Rizal, after having been duly sworn to in accordance with law, depose and state that:

1. That I am the actual buyer of a motor vehicle which described as follows:

CHASSIS NO.: MNCAXXMAWAGD40960
PLATE NO.: ACJ 6673

2. I am presently applying for the transfer of the aforesaid motor vehicle with the
LAND TRANSPORTATION OFFICE (LTO);

3. That the fact of purchase is evidenced by that Deed of Absolute Sale dated 17
October 2016 whereby WILSON TAN is the vendor and I am the vendee as entered in
the notarial register of Notary Public Delia C. Vivar-Dimaandal as Doc. No. 354, Page
No. 71, Book No. IX, and Series of 2016.

4. I hereby attest to the authenticity of said Deed of Sale and the genuineness of the
transaction and in furtherance thereof;

5. In my capacity as the buyer and new owner thereof, I hereby unconditionally

warrant the absolute title and ownership over the above described motor vehicle,
and I hereby declare that the same is free from any liens, suits, claims or
encumbrances whatsoever, or any matter which may prove adverse thereto;

6. I hereby release the Land Transportation Office, its officials, staff and personnel
from any suits, claims or liabilities which may arise from the approval of the said
application for motor vehicle registration or any process or matter pertinent thereto
and in the event of any suit, claim or liability, I hereby undertake to defend such title,
as well as assume and satisfy any such suits or claims resulting therefrom, without
cost, charge or liability whatsoever accruing to the Land Transportation Office, its
officials, staff and personnel;

7. I am executing this affidavit to attest to the truth of the foregoing facts, and warrant
the title and ownership of the above-described motor vehicle, and for whatsoever
legal intents and purposes it may serve.
