AFFIDAVIT OF LOSS

I, Chacha Cha, Filipino, of legal age, and resident of #716-A Almanza Metropolis
Condominium, Manila Doctors Village, Barangay Almanza Uno, Las Piñas City, after having
been duly sworn in accordance with law, hereby depose and say:

1. That I am a holder of a school identification card bearing serial number 2016400150


issued by San Beda College-Alabang School of Law;

2.. That sometime on December7,2017, before entering the school premises, I discovered
that my school/company ID card was no longer in my bag where I used to place it;

3. Hereunder are the facts and circumstances surrounding the loss of my Identification
card:
I was walking alone in a dark lit alley pondering on many I subjected myself to the
torture called law school when suddenly I was bumped into by a man who I first thought
was a thief. My bag fell and its contents splayed on the ground. I started picking my things
up when a pair of sneakers appeared before me. I looked up and saw the man who bumped
into me. He looked like a man that you would see in movies— tall, with broad shoulders,
his hips lean, his legs long, his dark brown hair gleamed even in the dull light of the alley.
It was thick and it was clear he washed it and let it fall where it lay for the part was natural
and not straight, it was swept back but some of it fell around his temples and curled a bit
around his ears. Right then, I knew I found the one. And so with the grace of a true Maria
Clara, I said to him “ Kuya ang gwapo mo.” He smiled at me and said “ Hindi naman,
sorry ha nagmamadali kasi ako may date pa ako.” With that, he turned and walked away.
I felt my heart broke into pieces. But life goes on. I finished picking my things up and
walk all the way home.
The next day, I was about to enter our school when the security guard asked for my
Identification card. I searched my bag for it but it was gone. Then, I remembered the
happenings the day before and thought that it might be in the alley where my bag fell.
Upon reaching the alley, I again saw the guy from yesterday and said “Hoy kuyang gwapo,
Kamusta date mo?”. He answered “Break na kami eh”. In an attempt to console him, I said
“ Ok Lang yan. Tulungan mo na Lang ako hanapin yung I.D. Ko”. He agreed and together
we looked for my I.D. That day I was certain that I lost something that could identify me
but also found someone who could change my identity.

4. I made diligent search to recover the said Identification Card but proved futile. For all
legal intents and purposes the said Identification Card is now considered lost and
beyond recovery.
5. That said school/company ID card was not confiscated by the authorities of company
by reason of any violation of company rules and regulations;

6. I am executing this affidavit to attest to the truth of the foregoing and for whatever legal
purpose/s it may serve.

SUBSCRIBED AND SWORN TO before me, this 18th day of February 2018. The affiant
Chacha Cha, whom I identified through her Driver’s License:, valid from August 2017 to August
2021 issued by the Land Transportation Office of Las Piñas City, personally signed the
foregoing instrument before me and avowed under penalty of law to the whole truth of the
contents of said instrument.
