The undersigned counsel for the Petitioner respectfully state that:

1. Petitioner Sumikat V. Galang passed away last October 6, 2018 at Livonia,

Michigan, Unites States of America.

2. It is hereby prayed that the heirs/children of Sumikat V. Galang namely:

Marogondon, Real Quezon, Philippines and at the undersigned counsel’s
law office at Donaal, Arciaga, Nacar and Partners Law Office, U/G floor,
Baden Powell Hotel, 26 Gov. Pack Rd., Baguio City, Philippines.

RELIEF

 WHEREFORE premises considered, it is prayed for the Honorable Court
to allow the heirs of the deceased Petitioner namely:

to substitute him as PLAINTIFFS in this case, and that all summons, orders,
notices, and other processes of the honorable Court be furnished them at
Centro B. Marogondon, Real Quezon, Philippines and at the undersigned
counsel’s law office at Donaal, Arciaga, Nacar and Partners Law Office, U/G
floor, Baden Powell Hotel, 26 Gov. Pack Rd., Baguio City, Philippines.

All other reliefs, just and equitable are likewise prayed for.
