TO : ALL MICRO COOPERATIVES

SUBJECT : REQUIREMENTS FOR ISSUANCE OF CERTIFICATE OF

COMPLIANCE (COC)

Pursuant to the provision of RA 6939 (Act Creating the CDA) and RA 9520
(Philippine Cooperative Code of 2008) and subsequently Memorandum Circular 2016-
07, the following are the REQUIREMENTS FOR THE ISSUANCE OF CERTIFICATE
OF COMPLIANCE (COC) TO MICRO COOPERATIVES, to wit:

A. Cooperative Annual Progress Report (CAPR) with three (3) ATTACHMENTS:

a. Financial Statements audited by:

i. External Auditor accredited by the Board of Accountancy;
ii. Audit Committee of the Cooperative;
iii. Audit Committee of the Federation/Union where the cooperative
is affiliated.

b. List of Officers and Mandatory Trainings Undertaken/Completed.

c. Social Audit Working Document (Annex A) and Social Audit Report

(Annex B).

B. Certification Fee of P 100.00

The COC issued shall cover the operations of the cooperative for the year
immediately preceding the reporting period and shall be valid and effective until April
30 of the succeeding year.

Micro Cooperatives however are required to encode their CAPR thru the web-
based system within 6 months from the issuance of COC.
