ROLEPLAY DISCHARGE PLANNING

Team leader : Titin Rahayu
Nurse associate : Okky Cintya P.D
Doctor : Adinda Dwi Karnita
Patient : Farikha N.M.S
Family : Bunga Innasofa

In the morning at Moro Mari Hospital, doctor Adinda visite the patient named Mrs.
Farikha. He told the head of the room that this day Mrs. Farikha can go home.
Doctor : good morning nurse Ayunda
Head of the room : good morning doctor Adinda.
Doctor : today my patient named Mrs. Farikha is allowed to go home because
the wound on her leg had improved. Please prepare the discharge
planning. This is the health program for Mrs. Farikha after being
discharged from the hospital.
Head of the room : yes doctor.

The head of the room tells the team leader to prepare the discharge planning document
for the patient.
Head of the room : nurse Titin, please prepare the discharge planning document named
Mrs. Farikha and this is the health program from doctor Adinda to Mrs.
Farikha.
Team leader : yes ma'am, I prepare it.

The team leader and the nurse associate prepare the discharge planning document for
the patient. After that the team leader told the head of the room and the head of the room

Head of the room : this is nurse Okky and nurse Titin who will explain about health
education program to Mrs. Farikha after discharge from this hospital.
Patient : yes nurse.
Team leader : I will explain about things to do after your discharge from the hospital.
Its about your diet that is low in fat, low in glucose. And you have to do
Regular exercise. Keep check and control your glucose and take your
medicine regularly. Any question ?
Patient : how about the wound care on my leg nurse ?
Nurse associate : I will explain about your wound care. keep the wound dry, Avoid
soaking your leg with hot water, Beware of signs of infection,redness
skin,the presence of pus, and the skin feels hot. Do you have any
question ?
