Affect/Mood

Anxiety Level: Mild Moderate Severe Panic


The client during the interview was receptive to greeting but had minimal verbal
output and spoke in a slow, low-pitched, barely inaudible voice. The patient had no
untoward signs of agitation and restlessness but had difficulty initiating and
maintaining eye contact.
Affect (Statement of emotion): Appropriate Inappropriate
Adequate Restricted ☑Blunted Flat Labile
The client’s affect appeared to be blunted upon observation. He showed poor eye
contact and had minimal verbal output characterized as slow, low-pitched, and
barely audible. When asked where he thought he was currently, he replied “Nasa
langit ako ngayon” in the same manner of speaking throughout the interview.
Mood (Prevailing emotional state): Euthymic Elevated
Depressed ☑Labile
Upon observation of the client during the interview, the nurse assessed him to have
a depressed mood. However, upon further consultation with the relatives, at home,
he had multiple manic episodes characterized by bouts of aggression and running
naked along the street while saying” Follow me, as I follow Pope. He is my
shadow”.

Insight: (Degree of awareness/understanding of problems and their causes)

Insight of the patient is poor. When consulted about the reason for his admission, he replied
“Nandito ako para tanggalin ang mga sumapi sakin”. He has also been having auditory and visual
hallucinations of the demon, his statement regarding being possessed might also be due to the lack
of vocabulary to express his problems. Although the patient’s insight is limited to belief of
possession. This belief however shows that the patient is aware that the symptoms he has been
experiencing were not normal.
Judgment
During the admission of the client, his response to queries about his judgement were intact
nevertheless, based from the patient’s history, one can conclude that his judgement was at one
point not intact. One week prior to admission, the patient’s mother saw him about to spray Baygon
inside his throat. This action shows that the patient’s ability to formulate decisions is impaired
especially regarding choices with possible impact on health and safety.
