MAN MACHINE INTERFACE - LEG INJURY

LOCATION: DAS ISLAND IMMEDIATE CORRECTIVE ACTIONS


INCIDENT DATE: 5th Nov 2017 9:40 am
INCIDENT DESCRIPTION: • Area Secured
• Safety Stand down conducted
While the operator was reversing the Manlift, the banksman • Investigation initiated
signaling slipped and lost his balance and the rear tyre rolled over
his leg, resulting in a fracture of lower right leg/ankle.

The IP was immediately taken to the DAS clinic and later


transported to Abu Dhabi for further treatment.

GENERAL RECOMMENDATIONS

• Always keep out of “LINE OF FIRE”. Take the time to think about the
possible consequences that may result from where we position
ourselves or the actions we perform.

• Be aware of the constantly changing work environment around you


and re-assess the risks whenever the job or conditions have changed.

• Safe driving policy to be reinforced. Refresher in-house training to be *Picture for illustrative purpose only

interface rules are communicated.
