I accept responsibility for the care and condition of the above listed equipment and will make arrangements to have the equipment
returned on the date and time stipulated; if the equipment is not returned on time, I will be billed for each extra day. I also agree to
securely store the equipment in a permanent building (rather than a tent) at any time while not in use. If the equipment is lost or
damaged, I acknowledge that I will be responsible for the replacement cost of new equipment. I realize equipment will not be
left or picked up at delivery location if a representative is not available to sign at agreed date and time. I agree that if I cancel my
rental within 14 days of the delivery date I will receive half of the rental charge back and if I cancel within 7 days of the delivery date I
will not receive a refund (if payment has not yet been received and the rental is cancelled, I will still be charged that amount). I agree to
submit a 25% non-refundable deposit for equipment.

HOLD HARMLESS PROVISION- LESSEE AGREES TO INDEMNIFY AND HOLD LESSOR HARMLESS FROM ANY AND ALL CLAIMS, ACTIONS,
SUITS, PROCEEDING COSTS, EXPENSES , DAMAGES AND LIABILITIES INCLUDING REAASONABLE ATTORNEY’S FEES ARISING BY
REASON OF INJURY, DAMAGE OR DEATH TO PERSONS OR PROPERTY, IN CONNECTION WITH OR RESULTING FROM THE USE OF THE
EQUIPMENT INCLUDING BUT NOT LIMITED TO, THE MANUFACTURE, SELECTION, DELIVERY, POSSESSION, USE, OPERATION, OR
RETURN OF THE EQUIPMENT INCLUDING BUT NOT LIMITED TO THE EQUIPMENT. LESSEE HEREBY RELEASES AND HOLDS HARMLESS
LESSOR FROM INJURIES OR DAMAGES INCURRED AS A RESULT OF THE USE OF SAID EQUIPMENT UNLESS LESSOR IS OPERATING THE
EQUIPMENT AND IS DEMMED BY A COURT OF LAW TO BE NEGLIGENT IN IT’S ACTIONS. LESSOR CANNOT UNDER ANY CIRCUMSTANCES
BE HELD LIABLE FOR INJURIES AS A RESULT OF ACTS OF GOD, NATURE, OR OTHER CONDITIONS BEYOUND ITS CONTROL OR
KNOWLEDGE.S

I UNDERSTSAND THAT BOUNCE HOUSES AND OBSTACLE COURSES ARE TO BE USED BY CHILDREN 12 YEARS OF OLD AND UNDER.
SLIDES CAN BE USED BY BOTH CHILDREN AND ADULTS

DUTY TO MITIGATE- IN THE EVENT OF INJURY, DAMAGE OR LOSS DUE TO LESSOR’S NEGLIGENCE, LESSEE AGREES AND ASSUMES THE
DUTY TO MITIGATE ALL COSTS RESULTING FROM SAID INJURY, DAMAGE OR LOSS.

DISCLAIMER OF CONSEQUENTIAL DAMAGES- BY SIGNING THIS CONTRACT, LESSEE AGREES TO FOREGO SEEKING ANY
CONSEQUENTIAL DAMAGES IN THE EVENT OF ANY INJURY DAMAGE OR LOSS DUE TO LESSOR’S NEGLIGENCE.
DISCLAIMER OF WARRANTIES- LESSOR MAKES NO WARRANTIES EITHER EXPRESSED OR IMPLIED AS TO THE CONDITION OR
PERFORMANCE OF ANY EQUIPMENT AND/OR PROPERTY LEASED BY LESSEE FROM LESSOR. BY SIGNING THIS CONTRACT, LESSEE
AGREES THAT ANY WARRANTY OF MERCHANTABILITY OR FITNESS OF A PARTICULAR PURPOSE ARE HEREBY DISCLAIMED BY SIGNING
THIS CONTRACT, LESSEE AGREES THAT NO EXPRESS WARRANTY AS TO THE CONDITION OR PERFORMANCE OF ANY EQUIPMENT
AND/OR PROPERTY LEASED BY LESSEE IS HEREBY DISCLAIMED.

Operator Safety Acknowledgement Form

Please initial next to each item:

 I understand that I am responsible for the equipment until the rental company arrives to pick it up.

 I understand that while children are in the inflatable equipment, a responsible adult must be supervising.

 I will ensure that all riders remove their shoes and any sharp objects before entering the inflatable. Sharp
objects include eyeglasses, toys, jewelry, and any item that could scratch the unit or another rider.

 I understand that food and drinks are not allowed inside the inflatable, including gum.

 I understand that no kind of spray string is to be used near the inflatable because it can severely damage the
vinyl.

 Unless the inflatable is specifically designed to be a water ride, the inflatable should not get wet.

 I will not allow riders to do flips, climb the netting or roof areas, or roughhouse in the inflatable. I will remove
riders who do not follow directions to ensure the safety of all riders.

 I will not allow riders to sit or play on the safety step.

 I understand that riders should be sorted by size to prevent injuries. Only children of approximately the same
size should be in the inflatable at any time.

 I agree not to exceed the recommended number of riders or weight limit. See rental agreement for specifics.

 Safety and evacuation procedures in case of unit deflation have been reviewed with me.

 I am aware of how the weather can affect the inflatable and I have reviewed the procedures and understand
what to do in the event of bad weather (rain or wind)

 I understand that riders must slide in a sitting position, feet first. Jumping and flipping from the top of the slide
is not permitted and can cause injuries.
