1. DOCUMENTS TO BE PRODUCED AT THE TIME OF REGISTRATION

(i) Copy of the Offer of Admission which was sent by SLIIT via post / e-mail.
(ii) Duly filled Enrolment Form (Form B- All 03 Pages) attached herewith.
(iii) Signed Indemnity Form (Form C).
(iv) Originals plus one A4-size photocopy of the G.C.E Advanced Level and the G.C.E Ordinary Level
Examination result sheets/certificates. If the A/L Result Sheet/Certificate is not yet available, please
provide a Printout of your A/L results as shown in the Department of Examinations Website
(www.doenets.lk)- Applicable only for Local A/L Candidates.
(v) The Original and one A4 size photocopy of the Birth Certificate.
(vi) Two Passport Size copies of a recent Colored Photograph with your Name with initials and the NIC
Number written on the reverse. One photo to be pasted on the Photo Scanning Sheet (Form D).
(vii) National Identity Card and an A4 size photocopy of the same.
(viii) The receipt of the payment for the First Semester Fee.

2. PAYMENT INSTRUCTIONS

i. Please make your semester payment, in full, in favour of “Sri Lanka Institute of Information Technology
(Gte.) Ltd.” at any branch of the below listed banks:

a. Bank of Ceylon A/C 1630552 (BOC e-banking interface for SLIIT is also available).
b. Sampath Bank A/C 003990000033 (Sampath Vishwa e-banking interface for SLIIT is also
available).
c. Nations Trust Bank A/C 100380005000.

ii. Credit Card payments will be accepted via the POS machines available at Malabe and Metro campuses
with a surcharge of 2.5%.

iii. For the Bank to accept your payment, please:

a. Use the SLIIT paying-in slip available at the Bank when making your payment.
b. Write your Student Registration Number, which is mentioned in the offer of placement, clearly
in all copies of the Bank Slips.
c. Ensure your Registration Number is correctly written.
d. Please produce the offer of placement to the Bank at the time of payment if necessary.

iv. Money Orders / Cheques will not be accepted.
