deposit slip through email together with the filled up Registration Form) with transmittal

undersigned. NOTE: Please bring with you the original or hard copy of the filled up Registration Form and the
original copy of the Deposit Slip during the day of the TEACHERS’ Congress.

Secretary. All payment by check must be issued to: Cebu City Private Schools Administrators’
Association, Inc. Please ask for Official Receipt for payment made at the Secretariat Office. No
pick-up service of collection is available. NOTE: Please bring with you the original or hard copy of the filled-up
Registration Form and the original copy of the Official Receipt issued on the day of the TEACHERS Congress proper.
