This is with reference to the letter that you sent to our client, Dr. Joan Clarence Morallo-Mempin,
dated January 22, 2019. In said letter you have requested Dr. Mempin’s office to issue you a
Certified True Copy of Acknowledgement Receipts pertaining to all the services you have availed
of at Clarence Slimming and Dermatology Center of which our client is the Operations Manager
and resident physician. Further, you alleged that such services availed of corresponds to an
approximate amount of Two Million Seven Hundred Thousand Pesos (Php 2,700,000.00).
Notably, you stated that this request shall be used as one of the supporting documents in
furtherance of your business venture, the House of Desiree.
As we are representing our client, we would like to inform you that all of the original records and
receipts pertaining to your request have already been endorsed to our law office. As can be easily
gleaned from the records, you have purchased treatment packages from January 17, 2014 to
March 24, 2018 with the total amount of Six Hundred Ninety Nine Thousand Two Hundred Ninety
Two Pesos (Php 699,292.00) paid through cash, credit card and checks. Additionally, we have
official records that reveal you having utilized some of the packages and different dates which
represents ________(Php_______). These documents were all properly recorded in the usual of
the course of business operations of Clarence Slimming and Dermatology Center which means
they have been signed by the authorized persons of the former and by yourself as its client.
Attached please find a summary of your transactions with our client.
It is basic in law that “he who alleges has the burden of proving his claim”. Therefore, it is
incumbent upon you to prove the amount that you have alleged as our client has her own official
records of your transactions with Clarence Slimming and Dermatology Center that rebut your
claim.
We will be anticipating your appropriate response on this matter .
