PAEP 16 –
Pursuant to Administrative Order No. 15-64, please constitute yourselves into an ad-hoc committee composed
of accredited Constructors Performance Evaluators (CPEs) to assess the above mentioned Infrastructure
project at UP Open University, Los Baños Laguna.
The Committee is expected to submit a fully accomplished CPEs report comprising the following:
A. Constructor General Information (name of Constructor/s, PCAB License Number, PCAB/ARC category
and type of contractor);
B. Project Data (project name, location, funding source, project cost, implementing office, duration of the
project, date started and completed, slippage, date of evaluation, and status of project);
C. CPEs Rating (workmanship, time, facilities environmental, safety and health, and resources
deployment, the CPEs Rating/ Final Rating and its Qualitative Description); and
D. Committee Action Request (CAR) duly signed by the Contractors and End – User.
The deadline for the submission of report is 12 December 2016.
All expenses of the Committee in the performance of its task shall be charged against the UP Open University
Budget.
The CPES Team including support staff shall be entitled to appropriate honoraria for the adhoc committees.
Payment of honoraria shall be on per “major” infrastructure project basis. “Minor” projects shall be lumped to
form one (1) major project. The Vice President for Administration, through the recommendations of CPEs –
System Implementing Unit Chair, shall determine the type of Infrastructure project evaluated for purposes of
payment of honoraria.
