With in week 4 of placement I will be able to Practice writing SOAP notes When my preceptor has
be write a SOAP note independently requiring and getting feedback from approved my SOAP note
no revisions with no error. my preceptor without any errors

By week 4 I will be able to understand the Working with my peers and When I see one of my guest
zones of regulation and be able to teach my to understand how to going into a yellow zone and
guest effective self regulation strategies such effectively to implement effectively using a self
as taking a break by using a visual timer or these strategies and to regulation strategies that will
reminding my guest to be using their thinking getting feedback from my help to deescalate the
brain and emotional brain. preceptors situation
By week 4 I will be able to write out a full Working on punctuality and Completing the activities on
daily planner independently with my guest being aware and conscious the daily activity plan
that is according to their therapy goals of timing on an activity
and being able to follow through
everything that is on the schedule

Guidelines:

o Goals should be developed and discussed with clinical supervisor after the first week of placement
o SMART objectives are specific, measurable, attainable, realistic & timely
o Students should have 3-4 learning objectives and should reflect core OTA/PTA competencies
o Students must submit this form completed and signed by the preceptor to the Dropbox at the END of placement
