AFFIDAVIT OF UNDERTAKING

That I, ______________________________ , of legal age, Filipino citizen and a resident

of ___________________, Tudela, Misamis Occidental, after having duly sworn to in accordance
with law, do hereby depose and say:

1. That I am the bondsman of accused EDDIE MANAPSAL, filed before the 5th Municipal
Circuit Trial Court (MCTC) Clarin, Misamis Occidental for Violation of P.D. 1602 docketed
as criminal case no. 6069;

2. That the recommended bail bond for the temporary liberty of the accused is
P 15, 000.00;

3. That as bondsman, I am putting up a cash bond in the amount of Php 15,000.00 for the
temporary liberty of the accused, per O.R. No. _______________ with the following
conditions:

a. The undertaking shall be effective upon the approval hereof and to remain in force at
all stages of the case until its final termination, unless the proper court directs
otherwise;

b. The accused shall appear before the proper court whenever so required;

c. That his failure to appear at the trial without justification despite due notices shall be
deemed an express waiver of his rights to be present on the date specified in the
notice, and in the same case, the trial may proceed in absentia;

d. That he will surrender for the execution of the final judgment;

4. That this affidavit is executed for the purpose of incorporating Sec.2 of Rule 114 of the
1985 Rules on Criminal Procedure.
