APPOINTMENT LETTER

I hereby appoint Mr. Jeremy Astrero as Quality Management Representative (QMR)


effective March 14, 2005. Duties and responsibilities include:

 Ensuring that the processes of the Quality Management System requirements


are established, implemented, maintained and sustained

 Reporting on the performance of the quality system to IPI management to


review and identify actions and opportunities for quality improvement.

 Coordinating with various internal departments or external body on matters


relating to the organization’s Quality System; and

 Promoting awareness on customer requirements.

I hereby appoint Ms. Aimee Varias as Deputy Quality Management Representative


effective June 19, 2006. Duties and responsibilities are:

 Ensure that the processes of the Quality Management System requirements


are established, implemented, maintained and sustained

 Report regularly to the QMR on the performance of the QMS and its needs for
improvement.

 Stand-in and execute the QMR task in the absence of the latter.
