First Stage

1. Reference is made to HQCAA letter No. HQCAA/2388/511/HR(Inq)/574 dated 09th October,

2018.
2. At this stage, it appears to the Board that you Engr. Hammad Ur Rehman (Sr. Deputy Director
Mechanical – In-charge of Package-09 IIAP Project) Witness No-5 is likely to be blamed for the
following;
a. For failing to take up/ resolve the matter of Rotunda pin assembly with PMC / contractor
after the defect was highlighted by Operations team.
b. For failing to take action through PMC on the proposal for resolution of rotunda pin
assembly issue forwarded by M/s ADELTE.
c. For failing to review the Operation & Maintenance manual for adequacy of information
provided therein as per contract agreement as no response was given to O&M manual
submittal and subsequent approval of draft O&M manual was deemed to be accorded.
3. You are informed accordingly and also apprised of your privileges under clauses (i) to (v) of
para 19.2 of CAAO 5-5 (revised). Kindly submit your response if any by Date positively

2. At this stage, it appears to the Board that you Air Cdr (R) Shahid Aslam Khan (Additional
Director project M&E) head of ORAT team Witness No-16 is likely to be blamed for the following;
a. For failing to conduct proper operational readiness test of PBBs in respect of PBB
technical parameters prior to operationalization of IIAP Islamabad as per requirement.
b. For failing to conduct proper operational readiness of PBBs in respect of trained PBB
manpower provision prior to operationalization of IIAP Islamabad.
c. For failing to review the Operation & Maintenance manual for adequacy of information
provided therein as per contract agreement as no response was given to O&M manual
submittal and subsequent approval of draft O&M manual was deemed to be accorded.
3. You are informed accordingly and also apprised of your privileges under clauses (i) to (v) of
