1. I have inspected the area / type of hazards and risks associated with above job
2. Measures taken:
No underground utilities like sewers, water pipelines, Electrical cables/wires.
Utilities like -------------------------------------------------------------------------------are in existence &
thoroughly barricaded with caution boards read as “keep off services” and explained to
excavating personnel.
Trenches, ducts, & hallow surfaces are adequately supported and covered.
Proper lighting is provided during night time.
Trenches /pits are adequately shored / timbered
Excavated areas have been provided with barricades fence.
Danger signs displayed near excavated area
Proper means of egress and access provided and marked
Water is being removed from pit/trench
Heavy equipment such as cranes, shovels, backhoe / dumpers etc are kept 6 m away from the
edge of excavated area.
At least 1.5m area earmarked from the edge of the pit to keep excavated material away from pit.
A person provided exclusively to keep strangers away from excavated area & to direct shovel.

Note: Site Manager to discuss along with clients rep in detail about real conditions of the excavated
area along with excavation engineer / foreman
- To prepare permit in triplicate.
- Original display copy to be displayed near excavation area.
- 2nd copy to be filed by HSE engineer.
- Permit can be issued for the time needed to finish the work.
- Daily inspection to be carried out by excavation engineer and HSE engineer and endorse on
permit.
- Any changes in site conditions would result in permit cancellation automatically.
- New permit to be issued after expiry.
- Only trained & experienced operatives to be used for excavation work. They shall wear proper
personal protection equipment needed for the site.
