Campference 2018 on November 1-3, 2018 at the Fellowship Baptist Church of Talisay, Inc., Teodora
Alonzo Street, Zone 8, Talisay City.

I am fully aware of and I agree with the rules set by those authority, particularly, the Campference
Director and Church Workers. I believe that this camp will help my son/daughter grow hostically and
will enable him/her to become a better person that God wants him/her to be.

I and my son/daughter fully submit to the camp policies, and give authority to the Campference
Director and Church Workers in responsibly and lovingly carrying out the corresponding actions based
on my child’s performance.
