To provide a standard procedure for discipline of persons for health and safety
infringements.

i) A verbal warning shall be given on first occasion
ii) A written warning shall be issued on second occasion
iii) Dismissal shall occur on the third occasion should the two previous
warnings be given within a six month period

4.4 Verbal Warning


The Supervisor shall bring to the attention of the Employee the incident which lead to the
disciplinary action the time, date, place and describe why it is seen as a breach.
The Employee shall be given an opportunity to explain the accident/incident and his/her
actions.
The Supervisor shall precisely explain the rules, regulation or procedures which have been
breached and reinforce to the Employee his duty to comply.
The Supervisor shall make it clear to the Employee what is expected of him/her in the
future.
The Supervisor shall indicate a period of time over which improvement is to occur.
The Employee shall be warned that any further breach within the time frame specified will
lead to further disciplinary action.
The Supervisor shall complete a "Discipline Procedure Report" and file it at the workplace
and shall inform the Employee of this action.
4.5 Written Warning
The same procedure is to be followed as outlined in section 4.4.
The Supervisor shall provide a copy of the written warning to the Employee. A copy of the
warning shall be held at the workplace.
Following either a verbal or second written warning, within a period of six months having
elapsed and no further warnings have been issued then the Employee will be seen to have a
"clear slate" and all records will be destroyed.
4.6 Dismissal
Dismissal, except in the case of serious misconduct, should follow the steps of a verbal and
written warning.
All investigations, statements, reports and previous warnings should be considered prior to
dismissal.
Once dismissal has been decided upon, it shall be executed in accordance with procedure
“Termination Procedure”. Close liaison with the appropriate union representative and/or
safety representative should occur and a staff witness should also be present when an
Employee is dismissed.
4.7 Site Instruction
A Site Safety Instruction shall be issued to subcontractors and/or subcontractor's
Employees should a breach of legislation or Company requirements be detected.
