Dear Participant:

The Committee of Employee Affairs wishes to assess the satisfaction of EMAPTA Versatile
Services Inc. employees regarding working conditions provided by the company as part of the
progress report submitted to the Company Head for the end of the year. As you are part of this
company, I am inviting you to participate in the survey for providing the data for this report.

The following questionnaire will require approximately 7 minutes to complete. It's content are
only focused on ratings based on individual perceptions of each employee on conditions on
completions of task, career concerns, efficiency of work environment, employee recognition,
company policies, relationship with colleagues and salary concerns. The results of this survey
will be used to enhance the work environment in order for it be more suitable for us employees.
The survey will not collect any identifiable information and no one will be able to connect your
responses to you.

If you wish to participate in this survey, please answer all questions honestly and return all
questionnaires promptly at the reception desk in your floor lobby. Participation is voluntary and
you may refuse to participate at any time. Your completion of this survey will serve as your
participation. Please keep this cover letter for future reference.

Employee Affairs.
