Undertaking

I, Mr/Miss Son/Daughter of
Resident of , do hereby undertake that:-
1. The information furnished in the application form and the documents attached with the application form
are correct and no information asked for has been concealed. That incomplete or incorrect information
will disqualify me for admission in the Foundation University Medical College (FUMC).
2. At any time, during the course of study, if it is found that any information provided by me was incorrect
or any of the documents produced at the time of admission was false/fake or tampered, which in it’s
original form, would have rendered me ineligible for admission under the rules of the FUMC, then my
name shall, subject to any other remedy which the Dean/Principal may like to seek, be immediately
struck off from the FUMC rolls. I shall not be entitled to object or seek any remedy for the aforesaid
decision of the Dean/Principal regarding my removal from the college rolls and that the decision of the
Dean/Principal /College Administration will be final and binding on me.
3. In case my name is struck off the college rolls, under clause (2) above, I will not be entitled to claim
refund of any of the fees/dues paid by me to the College for the academic year (January to December). I
also understand that the college dues once deposited will not be refundable under any circumstances
except security and hostel dues.
4. I shall attend more than 75 % of the specified lectures, seminars, clinics and practicals with effect from
the commencement of each academic year, failing which my name may not be sent to the university for
the Professional Examination of that particular year.
5. I can be stopped by the college authorities from appearing in the university examination of the
respective academic year on account of shortage of lectures as per the PM & DC rules, poor academic
performance, failure to pass the Send Up Test or inability to submit the prescribed “no dues certificate”
6. I realize that I will not be promoted to the next higher class, until I have cleared all the subjects of the
previous class of the MBBS course.
7. I will not indulge in politics or seek membership of any political party/organization/student federation
/union, nor will I attend any meeting of such party/organization/student federation /union. In case I am
reported to be guilty /involved in any of the aforesaid activities during my stay in the FUMC the
University / College authorities can rusticate me temporarily or expel me permanently from the College
and that I shall neither have any claim, nor will I go to any court of law against the decision of the
University / College authorities.
8. If ever a disciplinary action is taken against me by the University / FUMC authorities, I will not try to
create a law and order situation or instigate the College students against the action taken by the said
authorities.
9. I fully understand that in case there is any dispute between me on the one hand and the administration of
the FUMC on the other, regarding my involvement in a disciplinary or a political matter or regarding the
imposition of any penalty or damages on me, the matter shall be referred to the Dean/Principal as the
sole arbitrator and his decision in such capacity shall be final and shall not be called in question in any
court of law and shall be binding on me.
10. I will abide by the decision of the University/FUMC/authorities in case I am found guilty of ill –

 discipline, defamation, disrespect to an officer/staff of the FUMC or having committed other anti-
institutional activities.
11. I shall not stage incite, participate in or indulge in any walkout strike or other form of agitation against /
for the College, faculty members, officials or staff.
12. I fully understand that hostel accommodation will be provided to me, subject to its availability. In case
of non-availability of hostel accommodation, I will arrange the boarding on my own and the College
authorities will not be responsible for it in any case.
13. If I am provided hostel accommodation, I will abide by the hostel rules and regulations. I will only my
self reside on the basis of the Hostel Card issued to me by the FUMC authorities and will not hand over
the same to anyone else. In case of violation/abuse of procedure, I will deem myself as well as the
person, to whom the card has been illegally handed over by me, liable to strict disciplinary
action/cancellation of hostel card etc. I will vacate the hostel room as and when the relevant ceases to
operate or is withdrawn by the college authorities.
14. I have read the prospectus of the Foundation University Medical College and I am fully aware of the
requirements of its MBBS programme. I will abide by the rules and regulations of the FUMC and will
pay fee and all other dues regularly on or before the stipulated dates. In case of none compliance, I will
stand liable to be struck off the college rolls.
15. I agree to follow the rules and regulations of the FUMC enforced presently and hereafter, the new rules
to be made by the Government of Pakistan, the Pakistan Medical & Dental Council, the University or
the FUMC Authorities. I will not go to any court of law against the rules and regulations presently
enforced or hereafter, to be made by the authorities mentioned herein. I will do nothing inside or outside
the College premises, which may interfere with the administration and discipline of the FUMC.
16. I under stand that my failure to observe the above-mentioned undertaking would result in cancellation of
my admission/ expulsion from the College and that the decision of the University /FUMC in this regard
will be final and shall not be challengeable in any court of law.
17.* I solemnly declare that the financing for my education at the FUMC is based on the income of Father
/ Mother who is working abroad and getting remuneration in foreign currency. This arrangement will
continue throughout the course of my studies at the FUMC.

18. This is to certify that my son/daughter/ward named Mr/Miss
is applying for admission in the FUMC, with my permission and with full knowledge of the MBBS
programme. I undertake to arrange the payment of fees, fines, penalties, as and when levied and
all other dues to the FUMC on a regular basis and without delay.
19. During the stay of son/daughter/ward in the FUMC, whenever a change becomes inevitable in his /
her boarding status or any of the addresses/contacts already communicated to the FUMC, I will
myself visit the Manager Student Affairs and lodge such written request in person.
20. I will not request by any means for the change of the residential status of my son / daughter / ward
admitted against Expatriate / Foreign seats through out the entire degree programme.
21. I have read and understood all the clauses of this undertaking and I have made
Mr/Miss being my son/daughter/ward, fully wise and aware
about the provisions in and the implications of the clauses in this undertaking, contained in para 1 to
16 and guarantee that my son/daughter/ward, Mr/Miss
legally bound to comply with the clauses contained in para 1 to 20 of this undertaking, in letter and
spirit.
