Reconsideration;

the same issues and cause of action, that is pending with other
courts, tribunal or agency of the government; that I have not
theretofore commenced any action or filed any claim involving
the same parties, the same issues in any court, tribunal or quasi-
judicial agency and, to the best of my knowledge, no such other
action or claim is pending therein and that should knowledge of
a pending action is acquired after the filing of the complaint, I
undertake to notify this Honorable Commission within five (5)
