AFFIDAVIT

I, Bartolome Liga, Filipino citizen, of legal age, single, and a resident of 45 Malakas St.
Diliman, Quezon City 1100, after having been duly sworn in accordance with law, hereby depose
and say that:

1. I am employed as a Senior Traffic Bureau Officer of the Quezon City Traffic Bureau
since 1985.
2. On August 15, 2017, I was on duty at the Quezon City Traffic Bureau located at 53
Mindanao Ave. Unit 303 Ignacio Bldg. Quezon City.
3. Around 4:05 pm, I received a call from a traffic dispatcher at Mindanao Ave corner
Congressional Ave regarding a vehicular accident between a 10-wheeler truck and a
Honda Brio.
4. Upon receipt of this call, I, together with my team immediately responded and went to
the scene of the accident.
5. We arrived at the scene by 4:10 pm. Upon arrival at the scene, I immediately located
the accident and saw that the 10-wheeler truck is on top of a white Honda Brio crushing
it. I noticed that there were officers from the nearby Bureau of Jail Management and
Penology (BJMP) and First Responders trying to help the people inside the car get out.
6. My team and I after assessing the situation proceeded to manage the crowd and the
traffic situation. After the accident was cleared and the victims were brought to the
hospital, we proceeded back to the office to make our report.
7. As part of our report, we also reviewed the CCTV footage of the accident for the
cameras located at the corner of Mindanao Ave and Congressional Ave as well as the
one by Calia Lily St.
8. Our review of the CCTV footage by Congressional Avenue revealed the following:
a. That around 3:58 pm, a white Honda Brio with Plate No. AOA 3301 was traversing
the middle lane of Mindanao Avenue when about 5 meters from the corner of
Congressional Avenue it slowly moved to the left lane.
b. About 30 to 45 seconds after it safely moved to the left lane, a 10-wheeler truck
traveling in a speed faster than the normal speed prescribed in a local road hit graced
the left side of the Honda Brio, hit the center island and toppled over on top of the
Honda Brio crushing it instantly.
9. The review of the CCTV footage by Calia Lily St revealed the following:
a. Around 3:55 pm, a Honda Brio passed by the corner of Calia Lily St going towards
Congressional Avenue in the middle lane.
b. Shortly after, a 10-wheeler truck in the left lane passed by Calia Lily St travelling
in a fast speed without regard for the intersection.
