JOINT AFFIDAVIT

We, Eliza, Alfred, and Vincent all surnamed Ramos, respectively 12, 8 and 5 years of age
Filipino citizens, minor single, and a resident of 96, Maginoo St. Diliman Quezon City after
having been duly sworn in accordance with law, hereby depose and say:

1.) That affiants are all minor children of deceased Hercules Ramos and his wife, Mary Joy
Ramos.
2.) That deceased Hercules Ramos perished in a vehicular accident involving Topstar Mix
Ready Concrete
3.) That affiants were also inside the car when the incident happen.
4.) That affiants were injured because of the incident.
5.) That after the incident, affiants having been subjected through a traumatic experience are
having sleepless nights and anxiety thinking of the incident.
6.) That affiants have been undergoing psychological treatment to help them cope with the
difficulties brought about by the incident.
Further Affiant sayeth none.
