HOW TO REPORT A VIOLATION
HOW TO FILE A TITLE II OR TITLE III COMPLAINT

Title II applies to State and local government entities, and, in subtitle A, protects
qualified individuals with disabilities from discrimination on the basis of disability in
services, programs, and activities provided by State and local government entities.
Title II extends the prohibition on discrimination established by section 504 of the
Rehabilitation Act of 1973, as amended, 29 U.S.C. 794, to all activities of State and
local governments regardless of whether these entities receive Federal financial
assistance.

Title III prohibits discrimination based on disability in public accommodations. Private


entities covered by title III include places of lodging, establishments serving food and drink,
places of exhibition or entertainment, places of public gathering, sales or rental
establishments, service establishments, stations used for specified public transportation,
places of public display or collection, places of recreation, places of education, social
service center establishments, and places of exercise or recreation. Title III also covers
commercial facilities (such as warehouses, factories, and office buildings), private
transportation services, and licensing and testing practices.

If you feel you or another person have been discriminated against by an entity covered by
title II or III, send a letter to the Department of Justice, at the address below, including the
following information:

- Your full name, address, and telephone number, and the name of the party discriminated
against;
- The name of the business, organization, or institution that you believe has discriminated;

- A description of the act or acts of discrimination, the date or dates of the discriminatory
acts, and the name or names of the individuals who you believe discriminated; and

- Other information that you believe necessary to support your complaint. Please send
copies of relevant documents. Do n

HOW TO FILE A COMPLAINT WITH THE DAYTON HUMAN RELATIONS COUNCIL

If you believe you have been discriminated against because of your race, color,
religion, sex, ancestry, national origin, place of birth, age, marital status, familial
status, sexual orientation, gender identity, or disability in the areas of employment,
housing, public accommodations, and credit in violation of the City of Dayton Revised
Code of General Ordinances (R.C.G.O) 32.02-32.21 and 32.99, you may contact us to
discuss the filing of a complaint.

You may file your complaint by contacting the Human Relations Council (HRC) to
arrange an appointment to speak to an investigator, or you may visit the H
​ RC​’s office
between 8:00 a.m. and 5.00 p.m. to file complaints.
