PETITION

WITH ALL DUE RESPECT TO THE HONORABLE COURT,

petitioner through undersigned counsel, most respectfully manifests

that:

1. Petitioner is the father of minor , of legal age, married, Filipino,

with residence and postal address at Pawikan St., Poblacion

East, Lagawe, Ifugao Province, Philippines, where he may be

served with the processes of this Honorable Court, whereas

respondent is the local agency of the government charged with

the registry of birth, death and marriages in the Municipality of

Lagawe, Ifugao Province, Philippines, with official address at the

Municipal Hall, Lagawe, Ifugao, Philippines where it may be

served with summons and other processes of this Honorable

Court;

2. Petitioner registered his daughter JASMINE NGIPOL TOLETE

at the Local Civil Registrar of Lagawe, Ifugao on November 21,

 1994, a copy of the Certificate of Live Birth is hereto attached

and marked as Annex “A” to form part of this petition;

3. In said Certificate of Live Birth, Petitioner’s daughter’s sex was

erroneously entered as MALE instead of her true and correct

sex which is FEMALE;

4. That the foregoing error was not intended to conceal any crime

or evade any obligation whatsoever;

5. Petitioner now prays that the Certificate of Live Birth of his

daughter regarding her sex be corrected from MALE to FEMALE

in order to put in proper order her Certificate of Live Birth;

PRAYER

WHEREFORE, it is most respectfully prayed of this Honorable

Court that after due notice and hearing, an order be issued to the Office

of the Local Civil Registrar of Lagawe, Ifugao Province and the National

Statistics Office, Manila to correct the Certificate of Live Birth of

petitioner’s daughter containing her sex from MALE to FEMALE in order

to reflect her true and correct sex.

Petitioner prays for other relief, just and equitable under the

premises.
