BEFORE THE ASSESSMENT SCHEDULE

 I already checked the A) assessment center to ensure that the
supplies & materials are available, B) the tools and
equipment’s are in good condition and C) the supporting
documents are complete and also the safety requirements are
also checked.

II. INTRODUCTION
 Good Morning candidates, by the way I am RAFFY S. BESONIA, your
accredited competency assessor for SMAW NCII,
 and with me is ENGR. GLENN LLERIA, our TESDA Representative from
the Provincial Office. (purpose) Engr. LLERIA is here to observe the
Conduct of the ASSESSMENT whether it is in accordance to the
prescribe procedure from TESDA,
 so do not worry about his presence, he is only here to observe the
conduct of the assessment.
 So, may I have the assessment package ma’am, thank you? (GET THE
ASSESSMENT PACKAGE)

III. ATTENDANCE
 So, before we start our assessment proper, let me CHECK your
attendance first, kindly AFFIX your signature corresponding in your name
and kindly submit your admission slip confirmed that you are the
person taking this assessment.
(LET THE CANDIDATE SIGN AND COMPARE THE SIGNATURE FOR
DOUBLE CHECK.)
 So both signatures were identical with your admission slips, so meaning
you are really the person taking this assessment.

IV. ACCOMPLISHED APPLICATION FORM & SELF-ASSESSMENT

GUIDE
 So, I have here with me your accomplished application form and self-
assessment guide, I have noticed that you checked all the YES column in

 your self-assessment guide so meaning to say you are ready to take this
assessment and can perform all the competencies enumerated and
prescribed in this qualification.
 Today, I am going to conduct a competency assessment for SMAW NCII,
this is a qualification that has 3 competencies, the (4) Basic, (6)
Common and (1) Core Competencies, but the Basic and the common
competencies are already integrated in the core competencies so we
have to focus our assessment in Core competencies.
 This qualification it has ONE CORE COMPETENCY namely,
 (1) WELD CARBON STEEL PLATES AND PIPE USING SMAW. These
are the competencies that you are able to undergo in this assessment.

V.CONDUCT ORIENTATION
 So, the PURPOSE of this assessment is for me to be able to determine
the level of performance whether it meets the standards as per
stated in the Training Regulations.
 Another purpose for taking this assessment, because it is a requirement
of your EMPLOYMENT or your job PROMOTIONS or even for your
licensure exams or purposes.

VI. ASSESSMENT PROCEDURES

 (TIME FRAME) So, our procedures in conducting this assessment, the
first part is the Written Examination which will take 40 minutes, second
is the Actual Demonstration which will also take 3 hours and 20
minutes, so a total of 4 hours. So, after your demonstration I will ask you
some questions related to your performance and after that I will give you
feedback base on the result whether you are Competent or Not-Yet
Competent.
 (CONFIDENTIALITY) Don’t worry because the result of this assessment
will remain confidential, only you and me knows the result. This is to
maintain the integrity of the candidate.
 (ETHNICITY / LOCAL CUSTOM) If ever you need me to talk to you in
your vernacular or local dialect, please feel free to express.

 (LEGAL) Whenever you are not amenable with my decision, you have
the right to appeal. All you have to do is to right appeal letter addressed
to our TESDA Provincial Director (______________) and I, as your
assessor I am very much willing to recognize your complain but as much
as possible, I try my best that this assessment today all of us will agree
my decision.

VII. ASSESSMENT PROPER

 Okay so now your back, are you ready with your assessment?
 Kindly READ well the candidates guide, there are special instructions
 So, let us now proceed to our 1.) WRITTEN EXAMINATION.
- So read the instructions carefully, it is 40 items you will finish this in 40
mins., use the back of your questionnaire for your solutions. (Timer starts
now! Give the test questionnaire)

- So now, let us proceed on our 2.) ACTUAL DEMONSTRATION, so, this

is your CANDIDATES GUIDE please read the instructions carefully.
(Give the candidates guide) Are you ready? Okay so,
 CANDIDATE 01: Please proceed to the Unit 01.
 CANDIDATE 02: Proceed to Unit 02.
 Okay, so this is your tools, supplies, materials and PPE. Please observe
Safety Precautions at all times. (So, are you ready?) You are only given
3 hours and 20 minutes to perform the task competency on how to
WELD CARBON STEEL PLATES AND PIPES (Timer starts now..)
 NOTE: Use the Rating Sheet, observe and check the performance of
trainees.)

your seat and wait for your turn.)

 CHECK OUTPUT OF THE CANDIDATES
 CANDIDATE 01: (Check the output, give comments) Before I give you
feedback, first I will ask you some questions:
- SAFETY QUESTION Why is it necessary to wear a complete and
appropriate PPE while working?
- CONTINGENCY QUESTION How do you prevent electric shock?

VIII. FEEDBACKING
 This time we will now proceed to individual feedback…
 CANDIDATE 01: So based on the evidences gathered from your
performance on WELD CARBON STEEL PLATES AND PIPES, …you
are able to perform the task on competency standards although there
are some areas that you need to improve like ... So, next time you have
to practice more on … and also on the oral questioning you are able to
answer my questions successfully, so in this assessment you are
COMPETENT.
 (SIGNING) Kindly sign your CARS, and also the TESDA representative
sign, so, this is your CARS (Competency Assessment Result Summary)
 (INSTRUCTIONS) you can claim your NC II at provincial office 3 days
after this assessment so just bring your Passport size I.D, White
Background, with Collar, and 50 Pesos for processing fee. Before you
leave the assessment area, kindly perform 5S.
 (PERFORMANCE BASED) YOU MET THE STANDARDS, YOU ARE
COMPETENT.
 CANDIDATE 02:

- (FEEDBACK) So, based on the result of your WRITTEN TEST, you
scored 45%, your ACTUAL DEMONSTRATION, YOU WERE NOT
ABLE to FINISH WELD CARBON STEEL PLATES AND PIPES, and
YOU WERE NOT ABLE TO ANSWER SATISFACTORILY MY
QUESTIONS.

- (RECOMMENDATION) BUT IT WOULD BE BETTER next time since
you are not able to PERFORM CRITICAL ASPECTS OF THE
COMPETENCIES… this time, all you have to do is PRACTICE especially
in WELD CARBON STEEL PLATES AND PIPES according to standards,
study well the concepts…
