· अगर आप उत्तर प्रदेश के मूल निवासी हैं और आपिे इंटर की परीका यू.पी. बोर्ड से उत्तीर्ड की है तो मूल निवास प्रमार
पत्र भरिे की बाधयता िहीं है | ककसी अनय बोर्ड / प्रांत दवारा इंटर परीका उत्तीर्ड होिे की स्थिनत में यदद उत्तर प्रदेश में
निवास प्रदरश्डत ककया हो तो , प्रवेश प्रक्रिया के समय मूल निवास प्रमार पत्र प्र्ततुत करिा आवशयक है ।

· आवेदि पत्र के पप्रंट आउट पर मांगी गई कतुछ जािकारी अभयथि्थी जैसे बारहवीं के पूराणांक, प्रापतांक, भारांक , ््ीम ,
महापवदयालय का िाम एवं कोस्ड ्वयं भरेंगे तथिा सजस महापवदयालय में प्रवेश लेिा चाहते हैं, पवशवपवदयालय के दवारा
निरा्डररत अंनतम नतथथि के पूव्ड वांनछत प्रमारपत्र्रों को संलगि करते हतुए जमा करेंगे |

· एक अभयथि्थी एक ही पंजीकरर कर सकता है | एक से अथरक महापवदयालय अथिवा कोस्ड में आवेदि करिे हेततु, आवेदि
पत्र के पप्रंट आउट की फोटो कॉपी करा के प्रतयेक महापवदयालय एवं कोस्ड इतयादद ्वयं भरते हतुए पवशवपवदयालय के
दवारा निरा्डररत अंनतम नतथथि के पूव्ड वांनछत प्रमारपत्र्रों को संलगि करते हतुए सजस महापवदयालय में प्रवेश लेिा चाहते
हैं, जमा करेंगे |

· आवेदि में ककसी भी त्रतुदट के रलए आवेदक खतुद सजममेदार होगा|अतः आवेदि पत्र भरते वक़त एवं महापवदयालय में जमा
करिे के पूव्ड भली भांनत जाँच ले |

· अभयथि्थी यह सतुनिसशचत कर ले कक जो मोबाइल िंबर उसिे आवेदि फॉम्ड में भरा है वह ककसी भी महापवदयालय में
उसका प्रवेश समपतुषट (Confirm) होिे से पूव्ड बंद िहीं होिा चादहए तथिा उसकी पहतुंच में होिा चादहए ।

· ककसी भी महापवदयालय में प्रवेश लेते समय अभयथि्थी के ददए गए रसज्टर्ड मोबाइल िंबर पर एक OTP (One Time
Password) भेजा जायेगा जो कक महापवदयालय दवारा कंपयूटर में फीर करिे के उपरांत ही अभयथि्थी का प्रवेश समपतुषट
(Confirm) होगा । अतः सम्त अभयथथि्डय्रों को यथिा सूथचत ककया जाता है कक वे प्रवेश के समय रसज्टर्ड मोबाइल िंबर
के साथि ्वयं महापवदयालय में उपस्थित रहें ।
