- ​Rationale of the Research

Health status is a crucial aspects of all persons, whether in a


clinical setting, in public, or at home. Health status assessment within
the bars of a jail cell are very rarely done. Although they are at the
bottom of what we may call the social scheme or pyramid, considering
they are in fact, prisoners, they also live the same lives as everyone else
and experience similar physiological changes, specifically in bouts of
illness. Therefore, the qualitative study of health-related quality of life of
female prisoners is crucial, not only to undertake the scarcity of studies
covering this topic, but to develop public awareness of what truly goes
on inside the bars of the cell through a medical perspective

Nowadays, health has been defined as the ability of the body to adapt to
new threats and frailty and also defined in the presence or an absence in
disease. World Health Organization (WHO) defined health as “a state of
complete physical, mental and social well-being not merely the absence
of disease and infirmity.” This definition concerns for the individual as the
physical, mental and social functioning. This also shows that healthy
individuals are not suffering from any health related issues such as
diseases, or any hazards. Mental processes determine people’s
relationship with their physical and social surroundings, their attitudes
about life, and their interaction with others. It places health in the context
of environment, their health affected by everything they interact with- not
only environmental influences such as climate and the availability of
nutritious food, comfortable shelter, clean air to breathe, and pure water
to drink, but also other people, including family, loved ones, employers,
co-workers, friends, and associates of various kinds. This definition also
equates health with productive and creative living. It focuses on the living
state rather than on categories of disease that may cause illness or
death. ( Kozier et al., 2004)
“Health is wealth” an old popular saying or phrase that we usually hear
whenever we talk about the meaning of health this fits very well to any
individual. It means that health is the real wealth that gives strength of
our body and mind to cope and able to face any circumstances that
comes to our everyday life. Health is the most important thing for a better
quality of life without health we are unable to enjoy and meet the quality
of life we want to have and suffer from any difficulties.
Quality of life defined as an overall well-being of the person and
societies. It is also one of the most important considerations in every
individual especially in health care. Quality of life is the ability of the
person to enjoy normal life activities. Some medical treatments can
seriously impair quality of life without providing appreciable benefit,
whereas others greatly enhance quality of life.

Lucena City District jail Male/female dorm is the jail for both gender
but this study focus only on the health-related quality of life of female
inmates. Based on research of Mark Rod Labayo 2008; In US female
prisons, inmate society is generally made up of informal pseudo families.
Almost all inmates are part of a family and define their relations in
Kinship term. The pseudo family provides a predictable and stable
structure of social relations including homosexuality relation to which a
female inmate can turn for support. It is not uncommon, however, for the
different families to come in to conflicts.

Health care in the correctional system consists of local, state, and


federal jails and prisons, should be available and provided at the same
standard as health care in the general population. In this case, the
Supreme Court deemed that deliberate indifference to serious illness or
injury in a prisoners can be considered cruel and unusual punishment, in
violation of the eight amendment of the constitution. Cases following this
ruling have upheld three basic right: the right to access to care, the right
to care that has been ordered by a health professional, and the right to a
professional medical judgment.

Specific medical conditions are of particular concern due to their high


prevalence among incarcerated individual and the increased risks for
transmission of infection due to congregate living conditions. Since the
disease burden is high, correctional medicine may be more demanding
and costly than for non-incarcerated populations of similar age. In
addition to the high risk population served, the design of jails and prisons
and correctional health services themselves may unintentionally facilitate
the transmission of disease. Infection control n correctional facilities may
be adversely impacted by limited access to showers, a limited supply of

- ​Theoretical Framework

As the founder of modern nursing, ​Florence


Nightingale's Environmental Theory changed the face of nursing
practice. She served as a nurse during the Crimean War, at which
time she observed a correlation between the patients who died and
their environmental conditions. As a result of her observations, the
Environment Theory of nursing was born. Nightingale explained this
theory in her book, ​Notes on Nursing: What it is, What it is Not​. The
model of nursing that developed from Nightingale, who is considered

the first nursing theorist, contains elements that have not changed
since the establishment of the ​modern nursing profession. Though
this theory was pioneering at the time it was created, the principles it
applies are timeless. ​(Petiprin, 2016)

The focus of nursing in this model is to alter the patient's


environment in order to affect change in his or her health. The
environmental factors that affect health, as identified in the theory,
are: fresh air, pure water, sufficient food supplies, efficient drainage,
cleanliness of the patient and environment, and light (particularly
direct sunlight). If any of these areas is lacking, the patient may
experience diminished health. A nurse's role in a patient's recovery is
to alter the environment in order to gradually create the optimal
conditions for the patient's body to heal itself. In some cases, this
would mean minimal noise and in other cases could mean a specific
diet. All of these areas can be manipulated to help the patient meet
his or her health goals and get healthy.This theory is a patient-care
theory. That is, it focuses on the care of the patient rather than the
nursing process, the relationship between patient and nurse, or the
individual nurse. In this way, the model must be adapted to fit the
needs of individual patients. The environmental factors affect
different patients unique to their situations and illnesses, and the
nurse must address these factors on a case-by-case basis in order to
make sure the factors are altered in a way that best cares for an
individual patient and his or her needs. ​(Petiprin, 2016)

Nightingale’s Environmental Theory was the chosen theory for our


research since environment plays the important role in maintaining
the health, well-being and promoting recovery to the inmates with
acute or chronic illness. According to Nightingale in her theory there
is very strong relationship of a person with his/her environment,
health and nurse ​(Sher, et al., 2018). ​As a nurse we should find
ways to provide better health not only to our patients but also to
those person who need medical attention such as inmates with
diseases. To know if the inmates environment is the one factor or
cause of the disease. The environmental factor that affect the health
of the inmates includes: food supplies, clean water to drink, the
ventilation and lights in the facilities they are in and the noise they
heard from outside the cell. Those factors affect the prisoners health
not only their physical health but also their psychological health

The compilation of literature and studies that are applicable to the


present study focuses on the assessment of health-related quality of
life of Male Prisoners in Lucena, City District Jail Male/Female Dorm
Lucena City District Jail. These literature and studies will divided into
the following topics:Health Status and access to health services of
female prisoners in Greece: a cross- sectional survey; An
Examination of Care Practices of Pregnant Women Incarcerated in
Jail Facilities in the United States; The relationship between physical
activity and quality of life in prisoners: a pilot study; Cognitive
impairment, self-perceived health and quality of life of older
prisoners; Pregnant Inmates in Philippine Prisons: Securing and
Saving Them with Gender-Sensitive Policy Reforms; Quality of Life
Perception by Inmates in the Central Prison of Mbuji-Mayi,
Democratic Republic of Congo. studies that related in our research
are the following:

On Access to and utilization of health service.

Based on the study entitled ​“An Assessment of Female Prisoners’


Perception of the Accessibility of Quality Healthcare: A Survey in the
Kumasi Central Prisons, Ghana” Accessibility to healthcare for
females in prison around the globe is one of the major problems that
has made lot of attention among healthcare providers. Reviews of
related studies revealed problems in accessibility to healthcare
services for female prisoners. These deficiencies in health care
accessibility exist despite data confirming greater health care needs
among women inmates. revealed that prisoners considered health

services as part of their personal prison journey, which started on the
day of their imprisonment until their release. For those prisoners who
did not access health services outside the prison, imprisonment
improved their access to both mental and physical health
services.The officials rated the quality healthcare service and
accessibility to healthcare as poor indicating that one’s employment
status is likely to influence her assessment of the quality of
assessable healthcare. Females serving prison sentences gave the
highest rating in health assessment whereas those on remand gave
the lowest. ​(Sarpong, et al., 2015)
According to the study entitled ​“Health status and access to health
services of female prisoners in Greece: a cross-sectional survey​”
Most of the findings of literature related to this research are
consistent with the international literature. Prisoners has poor health
status compared to their previous health before entering in the cell,
as well as the increasing ratio of mental disorders and poor mental
health has been discussed in various studies. Half of the respondents
reported that they have poor access to healthcare services as well as
the quality of healthcare provided in prison was also poor. Another
study in Italy showed that prisoners with the average age of 40 years
graded their health as poor. Furthermore, the result of the study
regarding Greek female inmates’ point of view on the access to and
quality of health services also seem to be the same with other studies
reporting that inmates are provided a low level of inpatient care
during imprisonment, since they do not have a direct access to
healthcare in the community, as the general population, choose their
health service providers. In the European Union, female inmates are
more likely to suffer from sexually transmitted diseases (STD), due to
their high-risk behavior, such as prostitution, or traumatic
experiences, such as rape or sexual abuse. Despite the fact that
diagnostic tests are compulsory for prisoners according to the Greek
Penal Code, only 40 % of the respondents had been examined for

sexually transmitted diseases during detention. Similar findings have
also been reported in other surveys, highlighting the increased risk of
spreading these diseases to other prisoners and to the community
after release. ​(Geitona & Milioni, 2016)

Mental Healthcare
Most of the reviews related to this research, reveals that
approximately three (3) out of ten (10) female prisoners evaluated
their general health status as poor compared to their health status
before entering in prisons, while two-thirds (⅔) reported that their
health status are worsened during imprisonment, particularly their
mental health. More specifically, the rate of serious mental illnesses
among prisoners has been reported to be three(3) to five (5) times
higher than that of the general population. it means due to
imprisonment their mental illness got worsened. ​(Geitona & Milioni,
2016)
Given the inequality needs among women generally for mental health
support and care, very little has been made available in place of
detention for women in the Philippines. due to greater shortage of
psychological staff, it does not meet the care that the inmates needs.
some prisoners referred to the National Centre for Mental Health; this
reportedly tends to require a court order and severe symptoms of
mania. While one woman’s jail benefitted consistently from a staff
member with psychiatric training.​(Baker, 2011)

Care practice of pregnant inmates


pregnancy screening should be one of the most important steps in
providing quality health care to women being incarcerate. However, the
results of study entitled ​“An Examination of Care Practices of Pregnant
Women Incarcerated in Jail Facilities in the United States” suggested
that pregnancy screening is uncommon in jails: only few of jails reported

this practice. One possible reason for the lower rate of pregnancy
screening in jails (37.7%), compared with prisons (68.4%), is that women
in jail are sentenced for short periods of time than those in prison (Ferszt
and Clarke 2012). In conclusion, pregnant women who go to jail are less
likely to deliver in the facility than are those who go to prison. However,
given that this group of women is highly transient, for them this is the
only opportunity they have to receive proper prenatal care. Given the low
levels of pregnancy testing, the ratings of pregnant women who are
incarcerated will increasing continually (Clarke and Adashi 2011).
Concerted efforts to increase identification in jails throughout the United
States, especially in central and western regions, where testing rates are
lower, could greatly improve the health and welfare of both mothers and
children. Some surveyed facilities gave the pregnant women an options
as such adoption or termination. because there is only limited time to
care for their child because prison does not provide facilities for
breastfeeding and prisoners are not allowed to care for their child due to
lack of budget to provide the needs of the baby. Across correctional
facilities there were inequality in medical services provided due to lack of
budget in each inmates that why medical services does not give so
much attention. Almost one-third of the facilities did not have onsite
OB/GYN care. Instead, these facilities transported women outside of the
jail for medical care. If women are shackled during transportation to the
outside medical facility, they may be discourage them from going to the
medical appointment. Using restraints of any kind during daily routine at
the jail puts the mother and developing child at risk by increasing
susceptibility to falling and also increasing the risk for any complications
such preterm labor and hemorrhaging during delivery ​(D ​ eAngelis​. 2016)
Given the health risks posed and increased attention to the issue, it is
surprising that the rates of shackling in jail facilities are still so
high.​(Kelsey, et al., 2017)
Marital status influenced the mean health provision assessment ratings
of inmates,single female inmates rating are about 5.7 percent while

entitled ​“An Assessment of Female Prisoners’ Perception of the
Accessibility of Quality Healthcare: A Survey in the Kumasi Central
Prisons, Ghana”​ ​(Sarpong, et al., 2015)

common problems and diseases of prisoners


. The Penal Reform International confirms that the women in prison
comes from deprived backgrounds, and many of them has experienced
physical and sexual abuse, alcohol and drug dependence, and
inadequate health care before imprisonment.​(Sarpong, et al., 2015)
Life-term imprisonment and long-term incarceration produce varied
study, the life-term prisoners' perspectives of living as inmates in
problems for those serving such sentences are social isolation, total

being incarcerated, challenges of the prison environment, depression,
loneliness and loss of responsibilities combined with a regimented

Health related quality of life of prisoners:


Even if Health related quality of life (HRQoL) is rarely examined among
inmates population, physical activity helps to reduce depression and
anxiety experienced not only in female inmates but also in people
outside the cell, physical activity helps to increase functional capacity
and improving the health status of incarcerated people. Moreover, there
is evidence from the survey that BMI variations are inversely associated
to the HRQoL and tobacco smoking is inversely associated with the
mental component of HRQoL. The study results are promising and a

multicenter study is ongoing to expand the sample and include the
female population in the study. Further research approaches could
include the some interventions with the organization of physical training
and exercise in prisons and qualitative analysis to identify other
determinants of inmates HRQoL. If physical activity will be confirmed to
highly affect the inmates HRQoL practical implications have to be
considered. The realization of training programs for female inmates
could be simple, effective, and cost effective way to improve the Quality
of Life of prisoners and to contribute to the re-educational aim of the
prison detention. According with the Italian Constitution Document
“punishments must aim at rehabilitating the condemned” for the
restitution of healthy and renewed persons to the community ​(A.
MAnnocci et Al. 2015)

Physical health
Female prisoners in US who participating in the focus group reported
that limited physical activity affect their physical health. The related study
reveals that both the physical and mental components of the Health
related quality of life (HRQoL) in the inmates population are affected by
physical activity. According with ​Meek et al.​ we can state that physical
activity can contribute to achieving “healthy prison” objectives in practice.
Physical activity is effective tool to challenge chronic diseases in general
population. Physical activity programs in prisoners could contribute to
accomplish this target that seems to be the main determinant, joint with
the BMI variation and smoking attitude, of inmates’ quality of life...
Programs promoting physical activity will help in order to improve the
quality of life (QoL) of the inmates and allow a better social integration at

- ​Significance of the Research

This study envisions to benefit the inmates of Lucena City


District jail Male/female dorm. ​Firstly, so that the inmates can
stay healthy while inside the cell. Second, as a result of the
analyses of this study, they would figure out the implications of
the study to the inmates of Lucena City District jail
Male/female dorm. Findings of this study will also provide the
notions to other prisons to understand the importance of the
health status of their inmates

- ​Definition of Terms

To help the readers familiarize and enlighten the mind in reading


this study, the following terms are define appropriately in
alphabetical order as follows:

Acute - ​Of abrupt onset, in reference to a disease. Acute often also


connotes an illness that is of short duration, rapidly progressive, and
in need of urgent care.

"Acute" is a measure of the time scale of a disease and is in contrast


to "subacute" and "chronic." "Subacute" indicates longer duration or
less rapid change. "Chronic" indicates indefinite duration or virtually
no change. ​(Shiel jr., 2018)

Chronic - In medicine, lasting a long time. A chronic condition is one


that lasts 3 months or more. Chronic diseases are in contrast to
those that are acute (abrupt, sharp, and brief) or subacute (within the
interval between acute and chronic). ​(Shiel jr., 2018)

heredity, age, gender ​(Medialink, 2009)

Obesity ​- ​Obesity is a medical condition that occurs when a person


carries excess weight or body fat that might affect their health.

Quality of life -​The patient's ability to enjoy normal life activities. Quality
of life is an important consideration in medical care. Some medical
treatments can seriously impair quality of life without providing
appreciable benefit, whereas others greatly enhance quality of life.

includes having good mental health, high life satisfaction, and a

crime that they have committed. ​(Collins, 2019)

- ​ cope and Limitation of the Study

​The primary purpose of this study is to assess the health status of
female inmates, understanding their quality of life inside the jail and
how they cope with daily living. In order to gather the data needed in
our study, the researcher design will use a one-on-one interview for
each participating respondent. The respondents of the study will be
limited to ____ selected inmates. The data gathering will be
conducted in the premises of Lucena City District jail specifically their
personnel. The duration of the conduct of this study is approximately
one (1) year.

Limitations of this study are related with the setting, designs, data
collection and inexperience of the researchers. Research interview
will be conducted only within Lucena City District Jail with selected
respondents of approximately ten (10) persons. The duration of the
conduct of this study will be approximately one (1) year.

The desired outcome of this study is to identify the health conditions
of the inmates inside the specified jail. The analysis of data will
illustrate the common health problems of the inmates and the
treatment provided by Lucena City District jail
Male/female dorm personnel towards them. It will also provide
awareness and knowledge for the general public of what truly occurs
within the jail cells and the in-depth experiences of the inmates will

Research design

​ his study will utilize a qualitative approach to assess the quality of life of female

prisoners. Qualitative research is a type of social science research that collects and
works with non-numerical data and that seeks to interpret meaning from these data that
help us understand social life through the study of targeted populations or places.
People often frame it in oppositions to quantitative research, which uses numerical data
to identify large-scale trends and employs statistical operations to determine causal and
correlative relationships between variables. ​(Crossman, 2019). ​As a result of this study
is to assess the health status of the female inmates, understanding their quality of life
inside the jail and how to cope with daily living.

Research Participants

This study is to determine the quality of life of selected female inmates in Lucena,
City District Jail. The respondent for this study will be ten (10) inmates. The researcher
will explain the purpose of the study to the selected female inmates and understanding
the quality of life inside the jail, and how they cope with the daily living.

Research Instruments

There will be no research instrument that will use in this study. The researchers will use
the structured interview. A structured interview is when all questions are prepared in
advance. There are many ways in which you can conduct structured interviews. For
example you can conduct them over the phone, face-to-face, over the internet, using
computer programs such as skype, or using videophone ​(study.com 2019). The
researcher will record the interview using cellphone recorder.

Respondents/Units of Analysis/Subjects of the Study

This study is to determine the quality of life of selected female inmates in Lucena,
City District Jail. The respondent for this study will be ten (10) inmates. The researchers
will explain the purpose of the study to the selected female inmates and understanding
the quality of life inside the jail, and how they cope with the daily living.

Sampling Design and Procedures


The researchers will use a structured interview for the selected female inmates in this
study. This procedure will be employ to ensure appropriate representation of variant for
this study.

Interview will be employ a selected inmates in Lucena City District Jail. this structured
interview will base on the health related quality of life of female prisoners.

Research Population and Sample

The target population of this study is the selected female inmates of Lucena, City
District Jail. They were the right respondent for this study specifically to the purpose of
the study.

Ten (10) female inmates were selected to this study. The researchers will explain to the
selected female inmates the purpose of the study is understanding the quality of life
inside the jail, and how to cope with the daily living.

Research Locale/Study Site


The Capital of Quezon Province is Lucena, City. It covers approximately 84.03 square
kilometers or 8,402.66 hectares. This represents 9.6 percent of the province and 1.15
percent of the 2nd districts total land areas ​(Aron, Joma 2017).

Lucena, City had one (1) jail it is located at Dalahican road.

Data analysis plan


The interview will be conduct at Lucena, City District Jail Male/Female Dorm. With letter
requesting to conduct a study about Assessment of Health Related Quality of Life of
Female Prisoners will be send to Warden of Lucena, City Jail.

Research Paradigm/Conceptual Framework

As shown in the paradigm, the common problems that may encountered by the inmates
of Lucena, City District Jail. May include the insufficient medical care, quality of foods,
environment, overcrowding, and communicable disease.

Afterwards, female inmates should lessen the effects of these common problems in
their rehabilitation
