NARRATIVE OF ACCOMPLISHMENT REPORT

OPLAN BALIK ESKWELA S.Y. 2019-2020

The Cabantian National High School right after the conduct of Brigada

Eskwela, started its Oplan Balik Eskwela from May 27-31, 2019. Following a

steps to make the enrolment flow easy and systematic in order to serve the

stake holders with utmost convenience and fast transactions. The step is

Step 1 is the Assessment/Evaluation of CREDENTIALS – TLE/SPA/STE

Skills Assessment In charge. In this step all the documents were evaluated

and assessed to ensure that the documents presented are valid and

acceptable, also in this process the students were assessed regarding their

field of specialization/skills in their TLE. Step 2 is releasing/Filling of

Enrolment Form, after evaluation and assessment, the students were given

an enrolment form to be filled up. Step 3 is the checking of entry in the

enrolment form, in this process the form were checked by the teacher in

charge to ensure correctness of entries. Step 4, the enrollees were sent to

the clinic to fill up forms regarding health issue, to help the school clinic and

the advisers to have health condition background of the student. The next

step orientation, minor and major offenses were tackled and reviewed with

the students. The last step 6 is admission, in here, provided the student

followed the prescribed haircut and no long nails and hair colour, the student

is considered officially enrolled and admitted as a bonafide student of

Cabantian National High School.
