Initial incident notification: Verbal and/or written communication mode (e.g. phone, email) that
has to be performed within 12 hours after incident occurs.

b) Actual consequence (injury Æ recordable/non-recordable, Hours : 0-3
spill, property damage, etc), if any.
c) Immediate mitigation/action taken (e.g. IP treat, S/D, etc).
4.0 Incident Owner performs initial notification to:
• Field HSE Advisor.
• Field/Function Manager (e.g. Field Manager, Well Operations Hours : 3-6
Manager, Project Manager, Logistic Manager, etc).
5.0 Incident Owner and Field HSE Advisor assess risk rank of Incident
6.0 a) Incident Owner informs Risk Rank to Field/Function Manager
and HSE Manager.
b) Field /Function Manager perform initial notification to Sr.
Manager.
c) Field HSE Advisor performs initial notification to OHS
Hours : 6-12
Coordinator.
d) OHS Coordinator performs initial notification to HSE Manager.
e) Sr. Manager performs initial notification to Respective VP:
LOW/MED: By Email, SIGNIFICANT/HIGH (including
Recordable Incident): By Email and Phone.
7.0 Incident owner initiates formal incident reporting process (should be
posted in intranet within 24 Hrs) -
