AFFIDAVIT
I, HELEN P. TUSAÑEZA, of legal age, Filipino, widow with residence and postal
address at Lower Batinguel, Dumaguete City, Negros Oriental, Philippines, after having
been duly sworn to in accordance with law, under oath, hereby say THAT:

1. I am the registered owner of one (1) unit Stihl chainsaw, which is particularly
described as: MS-070 with Serial Number 814079862, which I purchased from JB
Marketing Inc. with business address at Dr. V. Locsin St., Dumaguete City, a legit
chainsaw dealer, evinced by a Permit to Sell Chainsaw No. PN07-DUM-PSCs-0004-2019.
A copy of the Permit to Sell Chainsaw, sales invoice and other pertinent documents to
support the legality of my purchased is hereto attached to form part of this affidavit to serve
as reference;

2. I am applying for a registration with the Department of Environment and

Natural Resources (DENR), CENRO II, Dumaguete City, Negros Oriental, for the
aforesaid chainsaw;

3. The chainsaw is needed for personal use and will be used for legal purposes
only;

4. I am executing this affidavit to attest the veracity of our foregoing statements

and for whatever legal purpose this might serve best.
