Greetings!

I, Andre Austin Ahmer O. De Castro with Lanbank Savings Account No. : __________________
hereby authorized Ms. Larryza Excija to perform the following on my behalf, specifically to;

1. Inquire on the mobile banking account details including but not limited to status of my
mobile banking app, the features it includes and the availability of the range of services
it offers.

2. Let her inquire “why does the fund transfer does not work on my mobile banking app?”
and list the possibilities of “what probably have gone wrong?” and how to resolve it?.

3. Please also allow her to execute request to activate the mobile banking feature of “Fund
Transfer” (if not yet activated in my account) which will allow me to conveniently transfer
funds to my account, to a third-party account, and to other bank soonest possible. (it
may be recalled that I already signed the Terms and Conditions of the Mobile Banking
last May 16, 2019 when I enrolled my account to Landbank iAccess).

Enclosed is my PRC License as proof of my authority for this transaction. I hope that this would
merit your prompt and favorable action.
