AFFIDAVIT OF EMPLOYMENT

I, MARK ANTHONY L. FABRICANTE, of legal age, Filipino, single, a resident of

Block 2, San Rafael, Guinobatan, Albay, after having been duly sworn to in accordance
with law, do hereby depose and states that:

1. I am a Culinary Chef/Assistant Cook by profession;

2. I was previously employed as Assistant Cook at CASA Dominga Restaurant

with office address at No. 8 Alegre St., Maharlika Highway, Balogo,
Sorsogon City from September 15, 2014 up to January 17, 2016, as shown in
the Certificate of Employment dated August 17, 2016 issued by the said
restaurant, copy of which is attached as Annex “A”;

3. I am executing this affidavit in connection with my application for

employment abroad; and

4. I attest to the truthfulness of this affidavit.
