permission to study at Don Honorio Ventura Technological University.

Enclosed herewith is a list of subjects I have already taken and also a list of subjects still
to be taken leading to the degree of Master of Arts in Education – Major in English duly
certified by the Dean/Registrar of this College. After the school term herein applied for outside
study I will submit to that Office a true copy of the report on the rating I obtained together with
the number of units earned duly certified by the Dean of Registrar of that College. My last
afternoon session in the Department of Education is 4:00 pm.
