RELEASE, WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

That I, ___________, of legal age, Filipino and with address at __________ do by these
present, acknowledge the receipt of the sum of __________ (PHP_______) from
CLARK INTERNATIONAL AIRPORT CORPORATION as full payment and
satisfaction of my unpaid salaries, wages, and other benefits due me in connection with
my employment at CLARK INTERNATIONAL AIRPORT CORPORATION.

That in consideration of said payment, I hereby release, waive, quitclaim and discharge
any and all actions of whatever nature, expected, real or apparent, which I may have
against CLARK INTERNATIONAL AIRPORT CORPORATION, including all its
directors, officers, employees, and agents by reason or arising from any and all money
claims, causes of action, or otherwise related to my employment with CLARK
INTERNATIONAL AIRPORT CORPORATION. Any and all actions which I or
jointly with others may have commenced before any office, board, bureau, court or
tribunal against CLARK INTERNATIONAL AIRPORT CORPORATION are
hereby deemed and considered voluntarily withdrawn, and I shall not testify or continue
to prosecute such action/s.

That, I have no more claims or rights of action of whatever kind and nature against
CLARK INTERNATIONAL AIRPORT CORPORATION and any officers thereof.

That in the event that LUZON INTERNATIONAL PREMIERE AIRPORT

DEVELOPMENT CORPORATION chooses to retain my employment, I accede to
restart my tenure and to be hired as a probationary employee of LUZON
INTERNATIONAL PREMIERE AIRPORT DEVELOPMENT CORPORATION
as a condition of my employment with LUZON INTERNATIONAL PREMIERE
AIRPORT DEVELOPMENT CORPORATION.

That I declare that I have read this document and have fully understood its contents. I
further declare that I voluntarily and willingly execute this Release, Waiver and
Quitclaim in a language familiar to me and is not intended to circumvent or defeat any
provision of law, rule or regulation.

That, I hereby attest that I fully understood all the terms and conditions of this Release,
Waiver and Quitclaim as well as the legal implications thereof and I hereby express my
complete agreement thereto.
