Guide for Processing BS

Note:

> All requirements must be photocopied and placed in a long white


folder. Except sea-projects.

> Sea-projects must be placed in a long brown and plastic envelope.

Requirements:

1. Clearance from the accounting

-Ask the accounting office a clearance for BS. Let them know that you're a BSM SNPL cadet.
For everyone's info, the BS enrollment is P27k+ for regular students. Ours is P35k
(shouldered by BSM)

2. Student's enrollment form copy

-Visit ma'am Josie at Campus Linkage Office and ask for the student enrollment form copy.

3. Transcript of Records

- We were given a copy of this upon graduating last 2015. If you lost your TOR, try asking the
registrar on how to get a new copy.

4. Notarized Company Sea-service

- CS is provided by the company. Ask for a copy from your supervisor or the assistant.
Afterwards, have it notarized. I suggest in front of the BSM House for only P50.

5. Company Evaluation

- Appriasals by the company

6. SIRB

- Seaman's book. From page 1, embarkation, to disembarkation

7. Crew list

- To be provided by those of us who continued the apprenticeship on a domestic/inter-island


voyage.

8. Ship's particulars from MARINA

- To be provided by those of us who continued the apprenticeship on a domestic/inter-island


voyage. This takes 3days to be released. So I suggest to get this earlier from MARINA. It
costs P400. All you need to bring is CS from domestic vessel.

9. Journal

- Daily Journal Book complete with the data required. Though they are not so strict in
checking the journal. This is not required by MARINA, but it is required by UC to process for
BS.

10. Contract of Employment (For foreign vessels)

- We were given a copy of the POEA contract prior to dispatch before joining our assigned
vessel.

11. Sea Projects

- Complete and original copies to be submitted as per TRB.

12. Passport

- First page only

13. Letter of Endorsement

- This was given by the STO during a seminar before we graduated last 2015.

After completing and submitting the requirements, you will then be schedule for BS
orientation seminar which is scheduled every TTH weekly.

First will be the CBT tests (POPEYE does not have tge answers for this test). You can review
at I-building CBT anytime as long as there's vacancy. Then, when you are ready, you can ask
the STO for a schedule to take the computer-based examination.

After passing th CBT test, there will be a Panel Interview at STO. The questions and answers
are on a PDF file "Panel Interview Notes". This just study the notes

When you're done with the panel interview, you will then be scheduled for final interview with
Dr. Robert Maluya the questions and answers for the interview are in PDF File "Final Interview
Notes" which is taken from the STCW Code. Just study the notes.

A valid Medical Result (2 years validity) is required before you can claim the BS certificate. If
you don't have a valid medical, you can ask for a referral from the STO. You will be referred to
UC-MED. This could also be used in review centers. Some if not, all review centers require a
valid medical. You will also need P25 for documentary stamp to be attached to the CS.

Note: If there's any confusion, everything will be explained during the orientation. You could
also ask questions.

Once your BS certificate is released from the STO, you can apply for with honors if your
grades qualify. Just coordinate with ma'am Josie.
Afterwards, prepare the following requirements to be submitted to the Registrar's office.

1. Apprenticeship enrollment form- Original

2. Logbook (TRB) endorsed by STO & Dean- Original

3. Notarized company sea-service- Original & 3 photocopies

4. Picture (2x2 & Junior size)- 1pic each with uniform & white background

5. Transcript Of Records- Photocopy

6. NSO Birth certificate- Photocopy

7. Seaman's Book (front page, embarkation, & disembarkation)- Photocopy

8. STO Endorsement- Registrar's copy

9. Yellow form (Application for Graduation)

10. Crew list (For domestic only)- photocopy

11. Ship Particular from MARINA- Photocopy

12. Contract of Employment (POEA)- Photocopy

13. Bridge/ Engine Watchkeeping (Original & 3 photocopies)
