I, JEOVIE R. SUCGANG, of legal age, Filipino citizen and a resident of

Mangan, Banga, Aklan, after having been duly sworn to in accordance with law, do
hereby depose and say:

That I am the registered owner and possessor of a motorcycle, particularly

described as follows:

That I have asked NELSON S. BAUTISTA, welder in Bautista Metal Craft

located at Nalook, Kalibo, Aklan, to attach a side car in the said motorcycle.

That I am executing this affidavit to include the sidecar in the registration of

the above-described motor vehicle with the Land Transportation Office and to
attest to the truth of the foregoing statements.
