AFFIDAVIT OF UNDERTAKING

I, _________________________________, minor/ of legal age, single/married and with residence address

at __________________________________, Capiz. After having been sworn to according to law, hereby depose
and say:

That I know that Hercor College Evaluating Package is viewed as a continuous process and therefore is
considered developmental on my part;

That required entrance examination for all applicants for freshman admission is for the purpose of assisting
enrollees to attain its goal in taking up the course and to provide better and quality education;

That I am an enrollee of HERCOR COLLEGE, Lawa-an, Roxas City, Capiz, and I am intending to enter
College of Criminal Justice Education specifically Bachelor of Science in Criminology;

Entrance Examination: That I am willing to undergo qualifications and examination systems providing for
individual entrance to the department and my qualification is dependent on the result of the examination;

That I am willing to comply all the school and department requirements particularly the norms of conduct,
discipline, academic and potential requirements and medical examination whenever required by the institutions in
accordance with the law;

Standing in the Department: That my status in the Criminology Department after admission is temporary
in nature, that should I fail to observe the grading standard and failed to make it at the end of semester necessary
for me to continue to pursue the course I am taking up then I voluntarily withdraw myself from continuing the
course and willing to shift to other course offered for by the school;

For the First to Third Year: That I was informed by the school authority that the minimum grade I should
maintain is a (GWA) General Weighted Average of not less than 2.25 every end semester. Passing grade based on
the General Weighted Average is a prerequisite for individual to proceed to another semester and year of the
program. Failure on my part to maintain the same then I covenanted myself to take another program;

Assessment, retention or withdrawal must always be supported by an official grades submitted to the Dean’s
office every semester and every year by a concerned instructor;

That I am executing this affidavit of Undertaking to attest the truth of the foregoing affirmation.
