I, Annabelle Fuentes Doroja, of legal age, married, Filipino with postal and
residential address at 23 A Unit 6 Arfel Street Arfel Homes Sanville Subdivision
1128, Quezon City, after having been sworn to in accordance with law do hereby
depose and state:

1. That I am presently unemployed and neither am I engaged in any kind

of business;

2. That I am executing this affidavit for purposes of applying for a Senior

High School voucher offered by the Private Education Assistance
Committee of the Department of Education;

3. That I am executing this affidavit to attest to the truth of the foregoing

and for whatever legal purpose this may serve;
