INTRODUCTION
Electronic Attendance Form is a easy way for
monitoring all Senior High School Students in attending
their class everyday. Attendance is very important in all
students, a single absent is a big difference in
performance in the school. Mostly students of senior
high are prone to absences, it is because of some reasons
that they think it is a boring class, laziness to attend the
class, some students prefer going to internet café playing
games rather than entering the class and some students
decided to stay in their home. Nowadays it is easy to
monitor their attendance unlike before the attendance
use a bond paper and put a check when the student is
present.

EXPLANATION OF THE PROBLEM

Our Proposal is Sta. Catalina National High
School wherein we introduce our project in the
Senior High Student our project is all about
Electronic Attendance Form, we decided to choose
this topic because of the occurring attitude on some
students who leaves and not attending the classes.
Some of students will be affected and it can also
cause of their failure. Many of the students who did
cutting classes and absenting doing this without
permission their parents. If there will be an accident
or problem happen when they doing this, and their
parents knows that they at school, it is a big
problem between the school administration and the
parents.

PURPOSE
The purpose of the study is to maintain the
monitoring the attendance of their class. To prevent
some students to cutting class and leave school
early everyday. This study also aims to increase their
grades and performance in the class.

OBJECTIVES
The objectives of the Electronic
Attendance Form are :
 To improve attendance as a means to increase
learning and student achievement in meeting
the state learning standards and reduce the
 dropout rate through the study of attendance
patterns.
 To accurately tract the attendance , absence,
and early departure of students to and from
school.
 To maintain the attendance of students was
recorded everyday.

TECHNICAL APPROACH

Step 1 : Check your Internet Connection

Step 2 : Go to google

Step 3 : Sign in your

google account
Step 4 : Go to google drive, select new then select
more then select google forms

Step 5 : Type your Form Title

ex. Attendance Sheet

Step 6 : Type the questions you woul'd like to ask

Step 7 : Beside the question there is a selection for
the answer, if it is short answer, multiple choice ar
even time.

Step 8 : For the last part of the Attendancesheet

they used to conduct are photo to upload that
prove that they are in school.
Step 9 : When its finish it automatically save
THEORY
According to Vantage software.com
attendance management becomes possible with
time and attendance monitoring system because
they allow the manager to be able to track
employee time in on efficient manner.

WORK PLAN
1. We want to spend atleast 3 days to create an
Electronic Attendance Form and have a example.

2. Then after we create a form we must be try to

some student and see if it is worked.
3. When it is good to them we make an approval
letter for the coordinator of SHS in Sta. Catalina and
for our teacher.
4. We make sure that every students who in and out
will automatically recorded to our Electronic
Attendance Form.
