Division Screening Guidelines for Athletes and Coaches

Rules on Participation
1. Participants
1.1 Participants in the Division Meet are elementary and secondary athletes from public and private
schools who have participated in the lower meet (Unit Meet).

2. Eligibility of Athletes
2.1 Must be a Filipino Citizen.
2.2 Must be enrolled in a recognized school at the beginning of the current school year.
2.3 Must have attended / finished the curriculum year.
2.4 Should have participated in the preliminary meets (Unit Meet).
2.5 Elementary athletes shall have been born in 2007 or later, and secondary athletes in 2002 or
later.
2.6 Pupil/student-athletes that transferred from one division to another are allowed to participate,
provided their transfer was done at the beginning of the school year. Transferees from one district to
another within the division are allowed to participate, provided, they have participated in the lower
meets.
2.7 Elementary athletes shall play in the elementary division, and secondary athletes shall play in the
secondary division. However, elementary athletes who been declared overage in the elementary
level are allowed to play in the secondary level.

3. Bases for Disqualification


3.1 Over-aged athletes.
3.2 Athletes with 3 failing grades in any subjects in the first grading period.
3.3 Athletes who are members of the National Team, national training pool and development pool
receiving monthly stipend / allowances from the PSC.
3.4 Athletes who failed to submit the required documents/failed to submit themselves for personal
interview on the specified time.
3.5 Penalized with suspension or ban during the previous Palaro
3.6 Elementary athletes with 3rd molars are automatically disqualified.
3.7 Athletes whose eligibility are questioned and pending appeal with the SDS are not allowed to
play.

4. Required Documents to be Submitted During Screening (All are Downloaded)


ID pictures of Athletes are printed in the AR-1 and Dental Certificate wearing the Orange Tshirt
same as last year (1.5”x1.5”) White background
ALL DOCUMENTS should be in A4 SIZE Bond PAPER
4.1 AR 1-Athlete's Record signed by the athlete, coach, and Sports Division Supervisor
4.2 Original and photocopy of Birth Certificate issued by the National Statistics Office (NSO) or
Philippine Statistics Office (PSA). Late registration may be accepted provided; the same was
issued one (1) year prior to Palarong Pambansa,
4.2.1. In the case of foreign born Filipino athlete, in lieu of NSO/PSA Birth Certificate, the
Original Birth Certificate issued by the country of his/her birth and a valid passport showing
his/her nationality, date and place of birth must be presented, or in its absence, an official
document from the Bureau of Immigration stating his/her citizenship.
4.3 Form 137, (REDUCED TO A4 before signing) computer printed, duly prepared by the teacher,
with fresh signatures of the - Adviser, Principal/School Head/Registrar, Division School Sports
Coordinator/SGOD Chief. Should have grades in the previous grade levels.
4.4 Certificate of Enrolment and Completion
Must be signed by the adviser and Principal/School Head/Registrar
4.5 Parental Consent – (Date must be before district Meet or Unit Meet) Must be signed by
parent. In case that one of the parents is deceased, write the name of the parent and remark
“deceased” beside the parent name: e.g. Juan dela Cruz - deceased.
If non-availability of parents, the guardian can sign provided that he/she will be able to present an
affidavit of guardianship.
Also bears the signatures of the adviser and principal/school head/registrar.In the box below, indicate
OFW,etc,( if)
4.6 Medical Certificate signed by a physician that the athlete is physically fit and with the date of
examination not later than three (3) months from the date of the opening program (The complete
name of the signing physician shall clearly appear on this document, with the license number and
date of examination.)

There is a separate/different Medical certificate for combative sports (2 pages) see downloaded
forms : Arnis, Boxing, Taekwondo, Wrestling, Wushu, Gymnastics, & Pencak Silat

4.7 Dental Certificate with a universal entry, duly signed by a dentist over his/her clearly printed
name and license number, and with the date of examination not later than six (6) months from date of
the opening of the Palarong Pambansa. Include signing Regional Meet. -Medical and Dental
check-up should be done by a Government Physician or Dentist.

5. Eligibility of Coaches and Chaperons


5.1 Coaches and chaperons must be school or DepED personnel.
Coaches and chaperons from private and public schools must have a minimum tenure of service for
at least six (6) months,
5.2 Documents to be submitted:
· Certificate of Employment
- Affidavit
- Sworn Statement
- Form 212 (revised 2017)
· Medical Certificate
- Certificate of Training
- Certificate of Membership
- Track Record

Other Requirements:
Necessary licenses or certifications including but not limited to, documents that will establish the
coaches’ competence, integrity and capability and legal relationship to the school and/or athletes to
show accountability.
Chaperons will submit Certificate of Commitment
Coaches should prepare 2 A4 Folders for (1) Gallery (1) Coach and Athletes Documents
Color of folder will be by team.
