ANNEXURE

A. The requirements of the customers and users, and of the provider of the laboratory services, including the
examination processes to be used, are defined as follows -
i. Laboratory offers a broad test menu comprising of approximately 210 tests and 70 profiles as
on date. We periodically review our test menu based on users feedback and survey.
ii. Laboratory advises to send min 2 ml sample for any individual test and 4 ml for any profile.
Reporting turnaround time for any test/profile is 48 hrs, however there are few low workload
parameters which are done on specific days, user can refer sample collection manual for more
details.
iii. Laboratory advises you to purchase Chiller boxes based on your expected workload for proper
transportation and maintaining cold chain.
iv. Laboratory insists that all tests and profiles should be from Thyrocare declared menu only, any
test/profile which is not in scope of laboratory will be rejected and returned to the user.
v. Laboratory can reject specimens incase of below deficiencies found
a) Inadequate volume
b) Samples not received in proper Pre analytical conditions (Clotted/ Hemolyzed/
Lipemic)
c) High TAT – All testing specimens should be received at laboratory within 48 hrs from
the collection time.
d) Ambiguity in work order or vial label
e) Vials received without barcodes
f) Usage of Non Thyrocare vials
g) Writing on barcodes of vial

vi. Laboratory insists proper patient information like clinical history/any medications/family
history etc for further clarity and records. Patient information is kept strictly confidential as
per our laboratory policies.
vii. Laboratory addresses any customer complaints/clarification/queries. Customers can be
advised to send an email on the below ids:
a) For Service – info@thyrocare.com
b) For Quality – labcomplaint@thyrocare.com
In addition we also have a dedicated panel of Pathologists and PHD’s who can address the
queries immediately and effectively.

B. Laboratory facility is spread in 34065 sqft and has the capacity to process more than 5 Lakhs investigations
per day and has adequate skilled technical staffs to carry out operations.

C. Laboratory has a panel of adequately skilled Pathologists/ PHD’s/ Technical staffs to perform intended
examination.

D. Laboratory performs preventive care tests and profiles and the same is declared as the scope of
Laboratory.

E. Laboratory informs all its users who are directly associated with us on any test’s cancellation/ rejection/
alert/ deviation observed through an email and SMS text.

F. Laboratory may refer examination to referral laboratory incase of major breakdown or technical issues
observed. However laboratory ensures that such incidences are not observed and it has sufficient back up
plans.
