II. CONTRACTOR’S CERTIFICATION

a. The Service Provider has fully completed the scope of work in accordance
with the plans specifications and other related contract documents by
furnishing all the necessary labor, materials, equipment, tools, supplies and
supervision (see attached individual Certificate of Completion)
b. The Service Provider has fully completed all the works as indicated in the
Program of Works within the agreed completion period.
c. The Service Provider certified the correctness of the data result submitted and

 d. willing to extend his services if necessary without any additional cost to be
collected in this Office after the acceptance of the project and within the
guaranty period.

III. ACCEPTANCE

We certify that the above mention project is 100% complete based on the
required PDE result stipulated in the contracts, plans and specification. We hereby
recommend for the acceptance of the project as described in Item I above conditions
provided under Item II.
