Suspect: 1. ROCKLEY CUROD y RELLOSA, 19 Y/O (DOB- July 30, 1999) single,
construction worker and a resident of Brgy. Lucong, Mabitac, Laguna. (Violation of Section
5 & 11, RA 9165)
2. RICHARD JAVIER y MORAN, 25 y/o, (DOB- October 1, 1983) single,
jobless and a resident of Brgy. Lambac, Mabitac, Laguna. (Violation of Section 11, RA
9165)
6. WHY/HOW:
That at about 1:50 in the afternoon of July 29, 2019, the above-captioned case
against the ROCKLEY CUROD y RELLOSA. was already filed by the undersigned
Investigation-on-Case before the Office of the Provincial Prosecutor, Sta Cruz Laguna
under the sala of Hon. Xavier Elbert V.Gordula. Assistant Provincial Prosecutor, Sta Cruz
Laguna docketed under NPS No. IV-04-INQ-19G-02068 to 69 for Sec.5 & 11 of RA 9165
while the case of RICHARD JAVIER y MORAN was docketed under NPS no. NPS No. IV-
04-INQ-19G-02070 for sec.11 of RA 9165. Further be informed that the Two (2) pieces
of confiscated heat-sealed transparent plastic sachet to ROCKELY CUROD y RELLOSA
containing white crystalline substance gave positive result for Methamphetamine
Hydrochloride, and weighing ALP-BB= 0.03; ALP-1= 0.03 grams as per Chemistry Report
No. LD-1564-19 and another one(1) piece of heat-sealed transparent plastic sachet
marked ALP-2 weighing 0.03 grams and it was confiscated to RICHARD JAVIER y
MORAN under Chemistry Report No. LD-1563-19 and both were gave positive result
and it was being examined by PSI ROBBIE CHARLES P VILLAGEN, Chief Chemist of
Laguna Provincial Crime Laboratory, Sta Cruz, Laguna. For info and Ref. BT.
