DETAILED REPORT OF INVESTIGATION

Metal slivers contaminant was intercepted at inclined magnet rod. Line was immediately stopped
and informed QA and Prep. QA AA. No affected crushed materials since diverted to juice from
start-up run until pau’ed. Possible source was undetermined as of writing since no welding activity
at preparation as per Prep. QA AA ANoay. Conforme: Processing Maintenance Staff – Socorin /
Processing Staff-RFDequito / CPCQA Staff-HPBurtanog.
